The TIF procedure rebuilds the valve between your stomach and esophagus without cutting into your body. Everything happens through your mouth.

The TIF procedure, short for transoral incisionless fundoplication, is an endoscopic surgery that repairs the anti-reflux valve at the top of the stomach. A surgeon passes a device through the mouth, folds the stomach tissue around the lower esophagus, and locks that fold in place with small fasteners. No abdominal incisions.

This article skips pricing, because insurance math shifts by plan and a number here would mislead you.

The Conditions TIF Was Built to Fix

TIF treats chronic GERD, mostly the heartburn and regurgitation that medication never fully settles.

Regurgitation is the symptom patients underrate. Acid and food climb back up the esophagus, sometimes into your throat at 2 a.m. A pill lowers acid. It doesn’t tighten a loose valve.

When acid keeps reaching the voice box, it becomes silent reflux, showing up as hoarseness and cough.

A small hiatal hernia usually rides along, and that detail matters more than patients realize.

How Does the TIF Procedure Work, Step by Step?

The whole thing runs under general anesthesia and usually finishes in under an hour.

  1. An endoscope and a specialized device go in through the mouth.
  2. Suction pulls the tissue where the stomach meets the esophagus.
  3. That tissue folds roughly 270 degrees around the lower esophagus.
  4. About 20 polymer fasteners lock the fold in place.
  5. The result is a longer, tighter valve, 2 to 4 cm.

Valve quality decides the outcome. A 2025 multicenter study in Gastrointestinal Endoscopy found acid exposure normalized in 94% of patients whose valve came out full-length and wrapped past 300 degrees, against 57% when it didn’t. Same device, different hands.

Almost nobody asks their surgeon how many of these they do a year. Ask.

Who Qualifies for TIF in 2026

Qualifying comes down to anatomy, not how badly you feel.

ASGE’s 2025 guideline suggests standalone TIF for confirmed GERD with a hiatal hernia of 2 cm or smaller and a Hill grade I or II valve. Bigger hernia or Hill grade III or IV, and it shifts to a combined repair that fixes the hernia first.

That 2 cm line is the most common reason someone gets told no. Not age. Not years of suffering.

I framed medication earlier as something that fails you. That’s not right. Plenty of patients in our Tampa practice respond fine to their pills and still want out. ASGE counts that preference as a reason to be evaluated.

Getting Ready for Your Procedure

Prep is short and dull, which is the point.

Solid food stops about two days out and clear liquids stop the night before. Blood thinners and diabetes medication get reviewed ahead of time. Arrange a ride home.

The part people rush is testing. Skipping pH monitoring or motility testing to get scheduled faster is how patients end up with a valve that was never going to help them.

What Does Recovery Really Feel Like?

Expect soreness for about a week and a restricted diet for four to six.

Chest and upper belly soreness is normal. Shoulder pain catches people off guard, and that’s just the nerves running to your diaphragm. Most patients drive and work at a desk within a few days.

The diet is what nobody explains properly. Clear liquids, then full liquids, then soft foods, then normal meals over two to six weeks. Bread and meat stay off the plate early, because dense food strains a healing valve.

Six weeks of soup is a real cost. Plan around it.

TIF Success Rates, and Where They Fall Apart

Most patients get off daily acid medication. Fewer stay off it for good.

ASGE’s evidence review found 77.6% of TIF 2.0 patients off PPIs at around six months, against 6.3% on medication or sham. Across 14 cohort studies covering 667 patients, only 28.6% were still on PPIs at an average of 19 months. Testoni’s 2024 work in Digestive Endoscopy counters that, with 53.3% off medication at three years.

Strong odds of relief. Moderate odds of permanence.

This is where I break with the marketing. A full wrap still controls acid better. A 2025 meta-analysis in Surgical Endoscopy showed traditional fundoplication cut acid exposure time more than TIF. TIF trades some of that for far less bloating and a preserved ability to burp. For a 1 cm hernia, worth it. For a 4 cm hernia, not close. A magnetic ring implant sits between the two.

How Risky Is TIF, Really?

Serious complications after this endoscopic valve repair are uncommon. Incisionless still doesn’t mean risk-free.

A meta-analysis of 781 TIF patients put serious adverse events at 2.4%, with perforation at 0.9% and bleeding at 0.65%. The 2025 multicenter cohort logged zero serious events across 87 procedures.

Set that against a 2026 Surgical Endoscopy review of FDA device reports, which counted 192 filings, perforation in 58% of them, and five deaths. That database has no denominator, so it can’t produce a rate. It still tells you which complication to respect.

Milder problems are more common. Sore throat, bloating, nausea, temporary trouble swallowing. Most clears inside a week.

The One Thing Worth Remembering

Treat this as an anatomy decision, not a symptom decision.

Patients who get measured properly, land inside the 2 cm window, and go to someone doing these regularly produce the numbers in the good studies. The ones who skip testing produce the bad numbers.

What is the TIF procedure, and how is it different from traditional reflux surgery?

Who does not qualify for the TIF procedure?

Anyone with a hiatal hernia larger than 2 cm or a Hill grade III or IV valve is not a candidate for standalone TIF. ASGE’s 2025 guideline points those patients toward a combined hernia repair with TIF instead. Major motility disorders and unconfirmed reflux also rule people out.

How long does TIF last before symptoms come back?

Durability varies more than most patients expect. Across 14 cohort studies covering 667 patients, only 28.6% were back on PPIs at an average of 19 months. At three years, Testoni’s 2024 data showed 53.3% still off medication, meaning roughly half restart something.

Can TIF be redone or converted if it stops working?

Yes. A failed TIF can be converted to a laparoscopic fundoplication later. The earlier repair changes the anatomy but doesn’t close the door on surgery.

Does TIF help silent reflux?

The evidence is stronger for classic heartburn and regurgitation than for throat symptoms. Some patients with hoarseness and cough improve, but a dedicated LPR trial was withdrawn before completion. Objective testing before any decision matters more here than anywhere else.

How soon can you eat normally after TIF?

Most patients return to a normal diet in two to six weeks. The progression runs clear liquids, then full liquids, then soft foods, then regular meals. Bread and meat come last, because dense food puts strain on a valve that hasn’t finished healing.

An endoscopy cannot tell you if you have reflux. It can only tell you if you have complications of GERD.
