Sugar by itself rarely sets off acid reflux. What sets it off is the company sugar keeps.

A donut brings fat. A soda brings volume. A slice of cake shows up at 9pm, forty minutes before you lie down flat. Cut the sugar, keep the 9pm habit, and most people feel exactly the same. I’ve watched patients do that for six months and come back angry.

Sugar and acid reflux are connected, but not the way most articles claim. Table sugar doesn’t loosen the valve at the base of your esophagus the way fat, chocolate, or peppermint does. Added sugar raises reflux risk through weight gain, through the fatty foods it travels with, and through when people eat it.

Does sugar make acid reflux worse?

Yes, but less than the internet suggests, and by an indirect route.

In a UK Biobank study of 167,574 adults published in the European Journal of Nutrition in June 2025, people drinking more than one sugar-sweetened beverage a day were about 7% more likely to develop GERD over a median 12.8 years. Seven percent. Real, and small.

Now a correction to what I just said. A randomized trial at Vanderbilt held calories steady so nobody lost weight, and the group cutting simple sugars still saw esophageal acid exposure drop 4.3 points while the control group’s rose 3.1. Same calories. Same weight. Different acid.

So sugar isn’t innocent. It just isn’t the headline.

Sugar and acid reflux prevention: what the evidence shows

Cutting added sugar lowers your risk. How much depends entirely on what you put in its place.

That same Biobank cohort found something the coverage skipped. Diet soda did worse than regular. Artificially sweetened drinks above one serving a day carried 12% higher GERD risk against 7% for sugar-sweetened, and swapping either for 100% juice dropped risk by roughly 9%. Patients switch to diet soda constantly and treat it as solved. It isn’t.

A separate study of 1,517 teenagers in Nutrients found the highest free-sugar consumers had about 66% higher odds of laryngopharyngeal reflux, the kind that hits your throat and voice instead of your chest.

The Dietary Guidelines for Americans released in January 2026 went harder than any edition before it. No amount of added sugar or non-nutritive sweetener belongs in a healthy diet, they say, with a cap of 10 grams per meal. One regular soda runs close to four times that.

How much sugar should you actually cut?

Start with what you drink. That’s where the volume hides.

The Vanderbilt group cut roughly 62 grams a day from a baseline of 116. Half. That’s not a trim, and half is what moved the pH numbers.

  1. Drop every sweetened drink, diet included.
  2. Read one label a day. Over 10 grams of added sugar per serving goes back on the shelf.
  3. Stop eating three hours before bed. Nothing to do with sugar, and it beats everything else here for most people.
  4. Log food and symptoms for two weeks. Not a month. Two weeks, kept properly, beats a year of guessing.

Heavy sugar loads also slow stomach emptying in some people, which keeps pressure sitting under a valve that’s already struggling.

The foods that calm reflux down

No food puts reflux out. Some just don’t start it.

Vegetables, non-citrus fruit, lean protein, oats, legumes. Fine list. What’s absent matters more: fat, volume, lateness.

This is where I push back on my own profession. The standard trigger list gets repeated everywhere as settled fact. The ACG guideline reads softer than that.

Carbonation is messier. A 2010 systematic review found no direct evidence that fizzy drinks cause or worsen reflux. A later meta-analysis tied moderate to heavy intake to higher GERD prevalence. Both stand. Run your own experiment.

What to do when cutting sugar isn’t enough

Eight weeks of hard sugar restriction and you’re still awake at 2am? The problem isn’t your diet.

I’m skipping medication dosing and reflux in children here. Both need their own article. What I’ll say instead: a hiatal hernia doesn’t care what you ate. Neither does a valve that has mechanically failed, and repairing the valve itself is a different conversation than reading labels.

That’s where the food articles stop. “Talk to your doctor,” and out.

Does cutting sugar lower acid exposure, or just make symptoms feel better?

Both. A randomized trial of 98 veterans published in the American Journal of Gastroenterology measured this directly with 24-hour pH monitoring. Cutting simple sugars by about 62 grams a day lowered esophageal acid exposure time by 4.3 percentage points, while the control group’s rose 3.1. Symptoms improved too, including heartburn frequency and sleep disruption.

Are artificial sweeteners safer than sugar for acid reflux?

No. In a UK Biobank study of 167,574 adults, artificially sweetened beverages above one serving per day carried a 12% higher GERD risk, compared with 7% for sugar-sweetened beverages. The 2025–2030 Dietary Guidelines for Americans state that no amount of added sugar or non-nutritive sweetener is recommended as part of a healthy diet.

How much sugar and acid reflux risk comes from drinks versus food?

Drinks carry more of the load because they deliver sugar fast and in volume. The Dietary Guidelines cap added sugar at 10 grams per meal, and a single regular soda runs close to four times that in one sitting.

Will honey or maple syrup trigger reflux the way table sugar does?

Evidence on honey for reflux comes from small pilot studies, not large trials, so calling it soothing overstates what’s known. All three are still added sugar and count toward your daily total.

Do I have to give up chocolate, coffee, and tomatoes?

Not automatically. In lab testing, chocolate, peppermint, alcohol, and high-fat foods lower pressure at the esophageal valve, while coffee, caffeine, citrus, and spicy food showed little to no effect. The ACG guideline notes that few studies demonstrate benefit from avoiding any of them. Test them individually rather than cutting all at once.

Does sugar and acid reflux research say anything about throat symptoms?

Yes. A cross-sectional study of 1,517 adolescents published in Nutrients found those consuming the most free sugars had roughly 66% higher odds of laryngopharyngeal reflux, which shows up as hoarseness, throat clearing, and chronic cough rather than heartburn.

When should I stop adjusting my diet and get tested?

When eight weeks of consistent change produces nothing, or when symptoms wake you at night, come with trouble swallowing, or involve unintended weight loss. Diet cannot correct a hiatal hernia or a mechanically failed valve, and continuing to experiment delays finding out which one you have.

An endoscopy cannot tell you if you have reflux. It can only tell you if you have complications of GERD.
