If coffee burns on the way down, you don’t have to give it up. Most people stop heartburn from coffee with four changes: a darker roast, a two-cup cap, food first, and nothing within three hours of bed. Those four handle it for most coffee drinkers I see.

Coffee is also a smaller villain than your search results suggest.

Heartburn from coffee happens when compounds in the brew signal your stomach to make more acid, and that acid washes back into your esophagus. Roast level, serving size, and what’s already in your stomach change how bad it gets. Adjusting those three stops the burning for most people.

Why Does Coffee Cause Heartburn?

Coffee triggers heartburn by pushing your stomach to make more acid. Its own acidity barely matters. Your stomach runs near pH 1.5 and coffee sits around 5, so pouring one into the other changes nothing.

What surprised me is how small the effect looks across whole populations. A 2026 review in Clinical and Translational Gastroenterology pooled 40 studies covering 122,074 patients. Reflux disease turned up in 34.9% of coffee drinkers against 30.7% of non-drinkers. The authors called that gap’s clinical importance unclear.

So coffee nudges your risk. It doesn’t create your reflux. If you already have chronic acid reflux, coffee is landing on damage that was already there.

Ways to Stop Heartburn From Coffee

Run these in order. The first three do most of the work.

Low-Acid Beans and Cold Brew Are Oversold

Skip both. “Low-acid” isn’t a regulated term, and the brands selling it rarely publish what’s in the bag.

Cold brew is the bigger myth. Thomas Jefferson University researchers who measured coffee acidity found cold and hot brews nearly identical, between 4.85 and 5.13. Cold brew tastes smoother because it pulls fewer flavor acids, not because it’s easier on your stomach.

I used to repeat the cold brew advice. The data didn’t back me up.

Does a Darker Roast Really Help?

Yes, and for a reason most articles get wrong. The benefit has nothing to do with acidity. A darker roast signals your stomach to make less acid.

University of Vienna researchers gave nine volunteers a dark roast and a medium roast with identical caffeine, then tracked stomach pH with a swallowed capsule. The dark roast stimulated noticeably less acid, thanks to a compound called N-methylpyridinium that builds up during roasting.

French, Italian, or espresso roast. Easiest fix on this list.

Will Decaf Fix It?

Probably not, and this is where the usual advice collapses. Caffeine was never the main culprit.

A JAMA study measured acid output after decaf and after a standard protein test meal. Decaf came out higher, 18.5 mmol per hour against 14.7. Stripping the caffeine doesn’t strip the compounds doing the work.

Decaf helps if caffeine itself bothers you. Otherwise it’s oversold.

How Much Coffee Is Too Much?

Two cups a day is the line to hold. Nurses’ Health Study II data on 48,308 women, published in Clinical Gastroenterology and Hepatology, found reflux risk climbed 34% at six or more servings daily. A later JAMA Internal Medicine analysis made capping coffee, tea, and soda at two cups one of five anti-reflux lifestyle factors.

One large mug on an empty stomach is worse than two small cups spread out.

Eat Something First

Coffee on an empty stomach is the worst way to drink it. Food gives the acid something to work on.

Reach for oats or a banana. Not because they neutralize acid, which they can’t. It’s the fiber. A fiber-rich diet improved symptoms in non-erosive reflux patients and raised resting pressure in the valve at the base of the esophagus.

Go easy on cream and syrup. Fat slows stomach emptying, which keeps acid sitting there longer.

Keep a Two-Week Log

Write down what you drank, how much, when, and what you ate with it. Two weeks shows the pattern.

Most people find the trigger wasn’t the coffee. It was the 20 ounces, or the empty stomach, or the 9pm cup. ACG suggests keeping food out of the three hours before bed, and coffee counts.

Watch for symptoms that aren’t heartburn. A raw throat, a nagging cough, or morning hoarseness can mean silent reflux.

The Changes Worth Making First

Make two changes and make them the roast and the timing. The rest is fine tuning.

Change How hard How much it helps
Switch to a dark roast Easy High
Always drink it with food Easy High
Cap it at two cups a day Moderate High
Nothing within 3 hours of bed Moderate Moderate
Switch to decaf Easy Low
Low-acid beans or cold brew Easy Low

When Coffee Isn’t the Real Problem

Run all six for a month and still burn? The coffee was never the issue.

I see this more than anything else. Someone spends three years fiddling with beans while reflux quietly damages the esophagus. The 2025 ASGE guideline puts reflux disease at about one in three American adults. Some have a mechanical problem no roast level touches, like a hiatal hernia, where part of the stomach slides through the diaphragm.

One line I’ll hold firm on. Twice-weekly heartburn for six months isn’t a surgery conversation. It’s a testing conversation. ACG recommends individualized trigger avoidance over blanket elimination, and that works only after testing shows what’s going on. For confirmed reflux medication can’t control, options run from incisionless repair to surgical repair of the valve.

You can usually stop heartburn from coffee with a darker roast, a smaller cup, and food first. If that fails, the coffee was just the messenger. Talk to our team in Tampa and find out why.

FAQs

How do you stop heartburn from coffee fast?

Switch to a dark roast, drink it with food instead of on an empty stomach, and cut back to two cups a day. Those three changes handle most cases within a week or two. If the burning keeps going past a month, the coffee probably isn’t what’s causing it.

Does cold brew really help with heartburn from coffee?

Not as much as the marketing suggests. Thomas Jefferson University researchers found cold and hot brews had nearly identical pH, between 4.85 and 5.13. Cold brew tastes smoother because it extracts fewer flavor acids, which doesn’t make it gentler on your stomach. A darker roast does more.

Is decaf coffee better for acid reflux?

Only a little. A JAMA study found decaf produced a peak acid output of 18.5 mmol per hour, higher than a standard protein test meal at 14.7. Caffeine isn’t the only compound in coffee that drives acid production, so removing it solves part of the problem at best.

How many cups of coffee can I drink with acid reflux?

Two a day is a reasonable ceiling. Nurses’ Health Study II data on 48,308 women found reflux risk rose 34% among those drinking six or more servings daily compared with none. A JAMA Internal Medicine analysis later used two cups as one of five anti-reflux lifestyle factors.

Does dark roast coffee cause less heartburn than light roast?

Yes, though the reason is often misstated. University of Vienna researchers gave nine volunteers dark and medium roasts with matched caffeine and measured stomach pH directly. The dark roast stimulated less acid production, because roasting creates N-methylpyridinium, a compound that reduces acid output.

Can I still drink coffee if I have GERD?

For most people, yes. A 2026 review of 40 studies and 122,074 patients found reflux disease in 34.9% of coffee drinkers versus 30.7% of non-drinkers, a small gap the authors said had unclear clinical importance. The American College of Gastroenterology recommends testing your own triggers rather than cutting coffee by default.

When should heartburn from coffee be checked by a doctor?

Get evaluated if heartburn hits twice a week or more for several months, if medication stops controlling it, or if you have trouble swallowing, unexplained weight loss, or vomiting. Roughly one in three American adults has reflux disease, and some of those cases involve a hiatal hernia or another mechanical problem that no coffee change will fix.

An endoscopy cannot tell you if you have reflux. It can only tell you if you have complications of GERD.

If you are unhappy with your reflux symptoms, come in and we can discuss testing and treatments that can accurately diagnose your problem.
