How long does heartburn last? Most episodes run 30 minutes to 2 hours. Mild heartburn clears in 15 to 30 minutes if you take an antacid and stay upright. Nighttime heartburn lasts longer, usually 1 to 3 hours, because lying flat takes gravity out of the picture.

But heartburn that shows up twice a week or more isn’t a bad dinner. It’s a valve that won’t close.

Heartburn is a burning feeling behind the breastbone caused by stomach acid flowing backward into the esophagus. A single episode typically lasts 30 minutes to 2 hours and usually begins within an hour of eating. Burning that happens twice a week or more points to gastroesophageal reflux disease rather than ordinary indigestion.

I’m a board-certified surgeon in Tampa and reflux is what I do all day. This article skips infant reflux and cardiac chest pain. Chest pain with sweating means call 911.

How Long Does Heartburn Last After Eating?

A typical episode starts 30 to 60 minutes after a meal and fades within 2 hours. Portion size and fat matter more than the item on your plate.

Your esophagus clears a splash of acid in roughly 99 seconds, but the burn lasts far longer. Irritated nerve endings keep firing after the acid is gone, which is why water never ends an episode.

Situation How long heartburn lasts
Mild burn, antacid, sitting upright 15 to 30 minutes
Big or fatty meal 30 minutes to 2 hours
Late dinner, lying down 1 to 3 hours
Untreated chronic reflux Months to years
Esophagus healing on treatment Roughly 8 weeks

Heartburn rarely travels alone. Watch for a dry cough, morning hoarseness, food catching on the way down, and waking at 2 a.m. for no reason. Mayo Clinic’s symptom list runs nearly identical.

Some people get all of that and never feel a burn. That pattern is silent reflux, and it goes unnamed for years.

Antacid tablet and water, the fastest way to shorten a heartburn episode

What Actually Shortens an Episode

Sitting upright plus a fast-acting antacid is the shortest path out. The rest is damage control.

In the first 30 minutes:

  1. Sit tall or stand. Gravity outperforms any pill early on.
  2. Take an antacid. Calcium carbonate and alginate products work in 5 to 10 minutes.
  3. Stop eating, and walk instead of lying down. Reclining restarts the clock.
  • Antacids cover 1 to 3 hours. Rescue only.
  • H2 blockers start in about an hour and run up to 12.
  • Proton pump inhibitors need up to 4 days of dosing. One swallowed mid-episode does nothing.
  • Potassium-competitive acid blockers are newer. Vonoprazan cleared FDA review in July 2024 and cuts acid within 1 to 3 hours.

Most people reach for milk instead. Actually, that framing isn’t quite right. Milk buffers acid for 20 minutes, then rebounds. A glass at 10 p.m. buys a longer burn at midnight.

Trigger Food Lists Are Mostly Overrated

The list you’ve seen a hundred times (citrus, tomatoes, garlic, onions, chocolate, coffee) carries less weight than everyone pretends.

Two major societies disagree in print. The American College of Gastroenterology suggests avoiding trigger foods. The American Society for Gastrointestinal Endoscopy, in its 2025 reflux guideline, names four lifestyle changes and trigger foods aren’t among them. Their four: lose weight, quit smoking, raise the head of the bed, stop eating 3 hours before bed.

Chocolate and coffee do relax the lower esophageal sphincter in a lab. But a meta-analysis of coffee drinkers found no real link to symptoms, even above four cups a day. Citrus and tomato sting an esophagus that’s already raw, which is irritation, not reflux.

So where should the effort go? The mechanics.

  • Going to bed within 3 hours of dinner raises the odds of nighttime heartburn roughly 7.5 times versus waiting four.
  • In one supervised weight-loss program, 65% of patients had reflux symptoms resolve completely.

I’ve watched patients cut nightshades for eight months and gain nothing. Then they move dinner from 9 p.m. to 6 p.m. and sleep through the night inside a week. The clock changes the outcome, not the diary.

And no diet repairs a mechanical problem. A hiatal hernia doesn’t shrink because you gave up marinara.

When Does Heartburn Stop Being Just Heartburn?

Two episodes a week is the line. Cross it regularly and you have reflux disease, not indigestion.

Claims data in the NIDDK’s 2025 burden report put GERD prevalence among privately insured Americans at roughly 11.1%. Globally, cases climbed from 450.8 million in 1990 to 825.6 million in 2021.

The threshold isn’t about how much it hurts. It’s about what years of acid do to tissue. Repeated irritation causes ulcers, scarring that narrows the swallowing tube, and in an estimated 5% of patients, Barrett’s esophagus.

Food sticking, pain on swallowing, unplanned weight loss, or black stools are alarm symptoms. Push for an endoscopy now.

Full after a few bites? Ask about gastroparesis.

What Happens When Medication Stops Working

Roughly 30% to 40% of people on long-term acid suppression never get full symptom control. Pills lower acid. They don’t close a valve.

This is where I’ll push back on my own field. You should have a pH study and manometry before anyone operates. Yet people reach Tampa Bay Reflux Institute after a decade of pills with nobody having checked whether their reflux is even acid. Skipping that workup is how a wrap fails.

Once the anatomy turns out to be the problem:

  • A fundoplication wraps the stomach around the esophagus to rebuild the valve. Reviews put long-term success near 80% at 18 to 20 years.
  • A magnetic ring implant holds the valve shut while leaving you able to burp. Pooled 2025 data showed about 89% off daily acid suppression.
  • An incisionless repair through the mouth fits hiatal hernias 2 cm or under.

A full wrap controls reflux best and brings the most bloating. A partial wrap trades control for fewer side effects. Ask how many your surgeon does a year.

Pregnant woman resting upright, since pregnancy heartburn lasts longer

Why Pregnancy Heartburn Lasts Longer

Pregnancy heartburn outlasts ordinary episodes because two forces stack. Progesterone relaxes the valve, and the growing uterus presses the stomach upward. Both run continuously, so nothing resolves once the meal digests.

It can start in the first trimester and worsens as pregnancy progresses. Twins or triplets make it worse. It ends when the pregnancy does.

Safe Relief While You’re Pregnant

Calcium carbonate antacids are the first choice. ACOG considers them safe at standard doses, capped near 2 grams a day.

Skip sodium bicarbonate. Half a teaspoon carries about 630 mg of sodium, enough to worsen swelling and blood pressure. Space antacids two hours from your prenatal vitamin, since calcium blocks iron.

Also skip the warm milk with honey. I know that’s what everybody repeats. Milk rebounds within half an hour, and honey has thin evidence at best. Peppermint tea works against you, since mint relaxes the valve progesterone already loosened.

Stop Timing It. Start Counting It.

Duration is a clue. Frequency is the diagnosis.

A burn that clears in 90 minutes after a heavy dinner is a meal problem. Heartburn twice a week for two years is structural, and no wedge pillow repairs structure. The patients who do best quit asking how long does heartburn last and started asking what keeps it coming back.

FAQs

How long does heartburn last after a single meal?

Usually 30 minutes to 2 hours, starting 30 to 60 minutes after eating. An antacid plus staying upright can cut that to 15 to 30 minutes. Large, high-fat meals push episodes toward the long end.

Why does heartburn last longer at night?

Nighttime heartburn commonly runs 1 to 3 hours because lying flat removes the help gravity gives in keeping stomach contents down. Going to bed within 3 hours of dinner raises the odds of nighttime heartburn roughly 7.5 times compared with waiting four hours or more. Raising the head of the bed works better than stacking pillows, which bends the body and increases pressure on the stomach.

How long does heartburn last if you have GERD?

With untreated GERD, heartburn keeps returning and can persist for months or years, because the valve problem behind it does not resolve on its own. GERD is defined as reflux symptoms happening twice a week or more. Claims data in the NIDDK’s 2025 burden report put GERD prevalence among privately insured Americans at roughly 11.1%.

How long does it take for heartburn medication to work?

Antacids begin working in 5 to 10 minutes and last 1 to 3 hours. H2 blockers take about an hour and cover up to 12 hours. Proton pump inhibitors need up to 4 days of daily dosing for full effect, so they do very little when swallowed during an active episode.

How long does it take for the esophagus to heal?

Esophageal lining damaged by acid typically heals in about 8 weeks once acid is controlled properly. Symptom relief usually arrives well before the tissue finishes healing, which is why people stop treatment early and relapse. Healing is confirmed by endoscopy, not by how you feel.

When should heartburn send you to a doctor?

See a physician if heartburn happens twice a week or more, persists despite over-the-counter treatment, or interferes with sleep. Get evaluated immediately for trouble swallowing, pain on swallowing, unplanned weight loss, vomiting blood, or black stools. An estimated 5% of long-term reflux patients develop Barrett’s esophagus, a precancerous change in the lining that is far easier to monitor when it is caught early.

An endoscopy cannot tell you if you have reflux. It can only tell you if you have complications of GERD.

If you are unhappy with your reflux symptoms, come in and we can discuss testing and treatments that can accurately diagnose your problem.
