Yes, Cialis can cause heartburn. About 1 in 10 men taking the 20 mg dose report indigestion in clinical trials, compared to 1% on placebo. That’s not a small signal. And the burn can stick around for a full day because tadalafil hangs out in your system longer than any other ED pill on the market.

But here’s the part most articles miss. If Cialis is lighting you up every single dose, the drug isn’t the whole story. It’s probably exposing something underneath. I see this pattern weekly at the clinic, and it’s the angle nobody talks about.

Let’s get into it.

Why does Cialis cause heartburn in the first place?

Cialis works by relaxing smooth muscle to improve blood flow. Problem is, smooth muscle isn’t just in your blood vessels. It’s also wrapped around the valve at the bottom of your esophagus, called the lower esophageal sphincter, or LES. That valve is supposed to stay tight so stomach acid stays in your stomach.

Tadalafil tells it to chill out.

A 2013 study in Gastroenterology found that a single 20 mg dose of tadalafil dropped LES pressure for up to 48 hours. Forty-eight. That’s nearly two days of a relaxed valve. For someone whose LES is already weak or who has a small hiatal hernia they don’t know about, this is when stomach acid starts climbing.

There are three things happening at once:

  • Your LES gets sloppy. Tadalafil relaxes it, so acid can slip up into the esophagus more easily.
  • Digestion can slow down. Some research suggests tadalafil slightly delays gastric emptying, which means food and acid sit longer.
  • The drug lingers. Tadalafil’s half-life is roughly 17 to 18 hours, with effects lasting up to 36. Sildenafil clears in about 5. So when Cialis triggers reflux, it doesn’t quit quickly.

In a head-to-head study of over 130,000 PDE5 inhibitor users published in European Urology Focus, reflux symptoms hit 8.0% of tadalafil users versus 5.3% of sildenafil users. Tadalafil wins on duration of effect. It loses on stomach side effects.

How common is Cialis heartburn at each dose?

Heartburn from Cialis is dose-dependent. The higher you go, the more likely you’ll feel it.

That data is from the official prescribing information on DailyMed. So at the highest standard dose, roughly 1 in 10 men get indigestion. At the lowest treatment dose, it’s still 4 times more common than placebo.

If you’re on daily 2.5 mg or 5 mg for BPH or daily ED use, the cumulative exposure matters. You’re not getting a 24-hour reprieve. The smooth muscle relaxation effect is constant.

Who’s most likely to get hit?

Some people sail through Cialis without a single burp. Others get nailed every time. Here’s what tips the scale.

You probably have a weak LES already. A lot of guys discover they have reflux issues only after starting Cialis. The drug doesn’t create reflux out of nothing. It usually unmasks a valve that was already underperforming.

You have an undiagnosed hiatal hernia. This is the big one most patients don’t know about. When part of the stomach pushes up through the diaphragm, the LES loses its mechanical support. Add tadalafil’s relaxing effect and you’ve got a reflux machine. I’ve sent dozens of men for upper endoscopy after they came in for “Cialis heartburn,” and a fair number walked out with a hiatal hernia diagnosis they’d had for years.

You’re already on a PPI and still symptomatic. If you take omeprazole or similar and Cialis still gives you heartburn, that’s a strong sign your reflux is structural, not just acid-related. PPIs reduce acid. They don’t tighten a loose valve.

You’re over 50. The LES naturally weakens with age, and gastric motility slows. Both make tadalafil’s effects more noticeable.

You’re carrying extra weight. Belly fat increases pressure on the stomach and pushes acid upward. Combined with a relaxed LES, the math doesn’t work in your favor.

What if it’s not just heartburn? Watch for silent reflux

Here’s something the ED telehealth articles never mention. Reflux doesn’t always feel like burning in your chest. Sometimes it shows up as chronic throat clearing, a persistent dry cough, hoarseness in the morning, or a feeling of something stuck in your throat. That’s silent reflux, also known as LPR, and it gets missed all the time.

I’ve had patients come in convinced they have allergies or post-nasal drip, and the real culprit was acid sneaking up while they slept, made worse by their nightly Cialis dose. If your throat symptoms started or got worse after starting tadalafil, that’s worth flagging.

5 ways to actually stop Cialis heartburn

Most lists you’ll read online give the same generic advice. Tums and weight loss and don’t lie down. Fine, but let me sharpen this.

1. Quit the worst triggers, not all the “triggers”

The internet will tell you to cut alcohol, coffee, chocolate, peppermint, citrus, tomato, spicy food, carbonated drinks, and probably joy itself.

Reality check. Research shows that the strongest evidence backs cutting alcohol, coffee, chocolate, peppermint, and fatty or fried foods. The data on spicy food and LES pressure is actually weak. Spicy food can irritate an already inflamed esophagus, but it doesn’t relax the valve.

So focus on the heavy hitters. Skip the wine with dinner the night you take Cialis. Push your morning coffee earlier. Don’t eat a 14-inch pizza two hours before bed and then dose. Those swaps move the needle more than going on a bland diet.

2. Stay upright for at least 3 hours after eating

Gravity is free. Use it. Stomach acid moves up easier when you’re flat, and tadalafil makes that even easier because the valve is loose. If you take Cialis in the evening, dose it earlier in the night so you’re not lying down on a full stomach with a relaxed LES.

For chronic reflux folks, sleeping with the head of the bed raised 6 to 8 inches makes a real difference. A wedge pillow works too. Stacking regular pillows doesn’t, because you bend at the waist and increase abdominal pressure.

3. Use antacids strategically, not constantly

Tums, Rolaids, Mylanta. These neutralize stomach acid for 30 to 60 minutes. They’re fine for occasional Cialis heartburn. The prescribing information notes that antacids may slow tadalafil absorption slightly, but they don’t reduce how much your body absorbs overall. So your Cialis still works. It just takes a little longer to kick in.

What I tell patients to watch for: if you’re popping antacids more than twice a week, you’ve crossed into chronic reflux territory. That’s a different problem.

4. Drop weight if you’ve got it to lose

This one isn’t sexy but it works. A study in Clinical Gastroenterology and Hepatology showed that losing weight reduces acid exposure in the esophagus and improves reflux symptoms in people with GERD. Even 10 to 15 pounds can shift the pressure dynamics in your abdomen.

The catch. Weight loss won’t fix a hiatal hernia. If you’ve got a structural issue, dropping pounds helps but doesn’t solve it.

5. Ask about lowering your dose or switching ED meds

If you’re on 20 mg and getting hammered, ask your doctor about dropping to 10 or trying daily 5 mg. Some men report fewer reflux issues on the lower dose. Others do better switching to sildenafil entirely. The case report in the American Journal of Gastroenterology documented a patient whose tadalafil-induced burning chest pain resolved after switching to a lower sildenafil dose.

Don’t change anything without talking to your prescriber first. But know that you have options.

How long does Cialis heartburn actually last?

For most men, several hours. For some, up to 24. The duration tracks with tadalafil’s half-life of 17 to 18 hours and total effect window of up to 36 hours.

Sildenafil-related heartburn typically resolves within 5 hours. That’s a meaningful difference if you’re picking between meds.

But if your heartburn from a single Cialis dose lasts more than 24 hours, or if it shows up between doses when the drug should have cleared, that’s not a Cialis problem anymore. That’s reflux you’d have anyway, and Cialis is just amplifying it.

When Cialis heartburn means you should see a reflux specialist

Here’s the line I draw in clinic. If Cialis heartburn happens once in a while and a Tums fixes it, no big deal. If it happens every dose and your weekends revolve around managing it, you need a workup.

Signs you should book an appointment with a board-certified surgeon who focuses on reflux:

  • Heartburn more than twice a week, with or without Cialis
  • Throat clearing, chronic cough, or hoarseness that won’t quit
  • Difficulty swallowing or food feeling stuck
  • Waking up with acid in your throat or sour taste
  • Already taking a PPI daily and still having symptoms

When should I see a doctor about Cialis-related heartburn?

See a reflux specialist if heartburn happens more than twice a week, doesn’t respond to antacids or PPIs, comes with throat clearing or trouble swallowing, or persists between Cialis doses. These are signs of underlying reflux disease that needs a proper workup.

An endoscopy cannot tell you if you have reflux. It can only tell you if you have complications of GERD.
