Yes, you can have acid reflux and never once feel heartburn. The acid reflux symptoms most people miss don’t show up in the chest at all. They show up as a cough that won’t quit, a voice that goes rough by dinner, or a throat you clear forty times a day without noticing. I’ve sat across from patients who spent six years on allergy medication before anyone looked at their esophagus.
By then the damage was already there.
What Is Acid Reflux, Exactly?
Acid reflux happens when stomach contents flow backward into the esophagus, the tube running from your throat to your stomach. A ring of muscle at the bottom is supposed to stay closed between swallows. When it weakens or opens at the wrong moment, acid and digestive enzymes travel up and irritate tissue with no defense against them.
Global cases climbed from roughly 451 million in 1990 to about 826 million in 2021, per Global Burden of Disease analysis published in 2025. North American prevalence estimates land between 18% and 28% of adults. Most of them assume ongoing reflux disease means burning, and that assumption keeps them undiagnosed.

Can You Have Acid Reflux and Not Know It?
Yes, and it happens more than people expect. When reflux travels past the esophagus into the throat and voice box, doctors call it laryngopharyngeal reflux. Patients know it as silent reflux, and many of them never feel chest burning at any point.
Your esophagus tolerates some acid. Your throat and vocal cords tolerate none. A fraction of the exposure causes trouble up there, which is why the chest stays quiet while the voice goes.
What that looks like day to day:
- Clearing your throat constantly, worst in the morning
- A hoarse or raspy voice that gets worse as the day goes on
- A lump-in-the-throat feeling with nothing there
- A dry cough after meals or at night
- Thick mucus, or postnasal drip that never drains
- A sore throat with no infection
Cleveland Clinic estimates more than half of people with chronic hoarseness have this form of reflux. Half. Most of them got handed an antihistamine first.
This article won’t cover what a repair procedure involves or which one suits whom. That needs test results, not a blog post.

Why Does Reflux Happen in the First Place?
Reflux is a mechanical problem before it’s an acid problem. The valve fails, or the anatomy shifts, and acid goes where it shouldn’t. What raises the odds:
- Extra weight carried around the midsection
- A hiatal hernia, where part of the stomach pushes up through the diaphragm
- Pregnancy
- Smoking
- Large meals, especially close to bedtime
- A stomach that empties too slowly
The Trigger Food List That Doesn’t Hold Up
This is where I part ways with almost every reflux article you’ll read.
That standard list of coffee, chocolate, citrus, tomatoes and spicy food rests on thinner evidence than anyone admits. The American College of Gastroenterology rates avoiding trigger foods as a conditional recommendation backed by low-quality evidence. Lab studies in their own review found coffee, caffeine, citrus and spicy food had little to no measurable effect on pressure in that lower valve. Their earlier guideline was blunter: cutting those foods across the board isn’t recommended treatment.
Two things do hold up, and they’re the two nobody bothers with. Weight loss carries a strong recommendation with moderate evidence behind it. In one tracked group, a BMI drop from 34.7 to 30.2 cut reflux symptom prevalence from 37% to 15%. Meal timing is the other. Eating within two to three hours of lying down raises overnight acid exposure.
I used to hand patients the full elimination list and tell them to add foods back one at a time. That framing was wrong, and I stopped. Most gave up their morning coffee for nothing while still eating dinner at 9 p.m.
How Do You Know If You Have Reflux?
Three steps, in order. The first costs nothing.
Watch Your Throat, Not Just Your Chest
Log three weeks of throat clearing, voice changes and cough, with the time of day each hits. Add what you ate and when you went to bed. Patterns you’d never catch in the moment show up fast on paper.
Test One Food at a Time
Skip the elimination diet. Pick the one food you most suspect, drop it two weeks, then bring it back and watch. Cutting twelve things at once tells you nothing, because you’ll never know which one mattered.
What Over-the-Counter Pills Actually Tell You
Antacids neutralize acid already in your stomach. H2 blockers cut how much you produce. Proton pump inhibitors lower production over several days and need consistent use first, so they do nothing for immediate relief.
The result matters more than the relief. Up to 40% of people with suspected reflux get inadequate control from PPIs. Eight weeks of correct use with no improvement isn’t a reason to double the dose. It’s information, and it points toward testing.

When Acid Reflux Symptoms Need a Specialist
Stop self-treating and get evaluated if any of these apply:
- Food sticking on the way down, or any trouble swallowing
- Over-the-counter medicine that’s stopped working
- Symptoms more than twice a week
- A family history of esophageal or stomach cancer
- Hoarseness or a cough that’s lasted more than eight weeks
My blunt opinion after years of seeing the back end of this: the people who end up worst off aren’t the ones with dramatic symptoms. They’re the ones whose symptoms stayed mild enough to tolerate and vague enough to blame on something else. Six years of throat clearing is not a quirk.

What if antacids and prescription pills stop working?

When should acid reflux symptoms be checked by a specialist?
Get evaluated if you have trouble swallowing, symptoms more than twice a week, medication that’s stopped working, hoarseness or cough lasting over eight weeks, or a family history of esophageal or stomach cancer. Delayed diagnosis is what allows long-term esophageal damage to develop quietly.
An endoscopy cannot tell you if you have reflux. It can only tell you if you have complications of GERD.
