Acid reflux after gastric sleeve is common, it often doesn’t fade on its own, and waiting it out is the wrong plan. A 2024 Obesity Surgery meta-analysis of 20,495 patients found reflux in roughly 35% after sleeve gastrectomy.

Acid reflux after gastric sleeve happens when stomach contents travel back up into the esophagus after sleeve gastrectomy. The reshaped stomach holds less and runs at higher pressure, while the valve above it loses strength. Around one in three patients develops it, often months or years after the operation.

Costs and insurance aren’t covered here.

What Is GERD, and Why Does a Sleeve Make It Worse?

GERD is reflux frequent enough to damage tissue or ruin your sleep. A sleeve worsens it by raising pressure inside a smaller stomach while weakening the valve above it. Ordinary reflux becomes chronic reflux disease once that lower valve weakens and stops closing.

Does Obesity Cause Reflux on Its Own?

Yes. Extra weight around the middle presses on the stomach and strains the valve above it, and a hiatal hernia can form as the stomach pushes through the diaphragm.

Then comes the line every patient hears: lose the weight and the reflux goes away. After a sleeve, it frequently doesn’t. Anatomy drives the problem now, not body weight.

How Common Is Acid Reflux After Gastric Sleeve?

Roughly one in three, and it climbs with time. Nearly everyone burns in the first weeks while the stomach tube is stiff, and most settles.

A 2025 British Journal of Surgery meta-analysis of 13,351 patients put new-onset reflux at 26.8%. On 24-hour pH testing rather than questionnaires, it hit 48%.

Actually, that framing isn’t quite right. Reflux after a sleeve isn’t one event. It builds.

Measure at 8 years Gastric sleeve Gastric bypass
Confirmed GERD 94.7% 25.0%
Erosive esophagitis 73.7% 18.8%

Those come from a World Journal of Surgery dual-cohort study. A pooled analysis of 19 studies found Barrett’s esophagus, a precancerous lining change, in 5.6% of sleeve patients.

What the Sleeve Does to Your Anti-Reflux Barrier

The operation cuts both ways, and which side wins depends on your anatomy and what got repaired.

Why Reflux Gets Worse

The new stomach holds about 150 mL and starts out stiff. Pressure inside rises while valve pressure drops. A sleeve that kinks or narrows at the angle forces content upward.

Why Some Patients Improve

The smaller stomach empties faster and makes less acid. Weight loss drops abdominal pressure, and the tube softens after about two years.

Can Your Surgeon Lower the Risk During the Operation?

Yes, and one decision outweighs the rest. Repairing a hiatal hernia during the sleeve changes the outcome dramatically.

A meta-analysis presented to a national surgical society found 78.4% of patients with reflux before a sleeve alone still had it after. Where the hernia was repaired at the same time, only 15.1% did.

The rest is technique, mostly avoiding a narrow spot at the angle.

Managing Reflux After Your Sleeve

Start with food and sleep, move to medication, then get tested before anyone discusses surgery.

Food and Drink

The photocopied list of forbidden foods is mostly folklore, and I’ll defend that. The 2022 ACG guideline recommends against eliminating trigger foods across the board, and backs cutting only what you’ve personally identified. In lab testing, coffee, caffeine, citrus and spicy food had little to no effect on valve pressure.

Quantity holds up better than any blacklist. Nurses’ Health Study data linked six daily servings of coffee, tea or soda with more reflux than zero.

Daily Habits

Raise the head of the bed. Stop eating two to three hours before bed, which the ACG recommends for nighttime symptoms. Loosen the waistband. Quit smoking.

Medication

Proton pump inhibitors are first-line and work better daily than during flares. But “stay on them forever” isn’t a plan. Registry data in 2025 reviews put acid-blocker use after sleeve near 37%.

Procedures and Revision Surgery

Test before you operate. This is where most patients get failed.

Plenty get labeled refractory without ever having pH testing or manometry. In my clinic, testing comes first. It shows whether acid actually reaches the esophagus and whether silent reflux is involved.

Then options open. A surgical wrap from the top of the stomach. A magnetic device at the weakened valve. An incisionless repair through the mouth. Conversion to gastric bypass.

Should You Have Had a Bypass Instead?

If you had meaningful reflux before surgery, probably. Society guidance updated for 2025 suggests medical therapy first, then bypass conversion above a BMI of 35.

A 2024 Surgical Endoscopy meta-analysis found sleeve patients far likelier to need revision surgery, odds ratio 11.47. Below a BMI of 35 the comparison gets murky, and a magnetic device may serve you better.

Acid Reflux After Gastric Sleeve Is Worth Fixing Properly

Get tested. That’s the one thing to take. Years of unmeasured acid is how a manageable problem becomes Barrett’s, and the patients I meet who wait longest have the fewest options left. Acid reflux after gastric sleeve responds well once somebody measures it. Tampa Bay Reflux Institute helps you eliminate reflux and GERD, and Dr. Gopal Grandhige is a board-certified surgeon who starts with real data instead of guesswork.

FAQs

How long does acid reflux after gastric sleeve last?

Most patients burn for a few weeks after surgery while the stomach tube is still stiff, and that early phase usually settles on its own. Reflux still present at twelve months tends to stay. A five-year cohort published in 2025 found GERD rising from 16% before surgery to 64% afterward, so time alone works against you rather than for you.

Will losing more weight fix my reflux after a gastric sleeve?

Often not. Weight loss helps reflux in people who haven’t had surgery because it lowers pressure inside the abdomen. After a sleeve, the driver is anatomy: higher pressure in a narrow stomach, a weaker valve, and sometimes an unrepaired hiatal hernia. Those don’t shrink along with the scale.

Can acid reflux after gastric sleeve lead to Barrett’s esophagus?

Yes. A pooled analysis of 19 studies found new Barrett’s esophagus in 5.6% of patients after sleeve gastrectomy, with rates climbing the longer patients were followed. Barrett’s is a change in the esophageal lining that raises cancer risk, and it causes no symptoms of its own, which is exactly why monitoring matters more than how you feel.

Is gastric bypass the only fix when medication stops working?

No, though it stays the most reliable option for severe cases, with remission near 80% in published series. Repairing a hiatal hernia, building a surgical wrap, placing a magnetic device at the valve, or doing an incisionless repair through the mouth all work in carefully selected patients. The right choice depends on hernia size, how your esophagus moves, and your BMI.

What tests should I have before anyone recommends revision surgery?

Upper endoscopy, pH and impedance monitoring, and esophageal manometry. Endoscopy shows damage, pH testing confirms acid is actually reaching the esophagus, and manometry measures how the muscle moves. Skipping these is the most common mistake I see, because symptoms alone correlate poorly with what testing reveals.

Can I drink coffee again after a gastric sleeve?

Possibly. In laboratory studies, coffee and caffeine had little to no measurable effect on lower esophageal valve pressure, and the 2022 ACG guideline recommends against eliminating trigger foods across the board. Nurses’ Health Study data did link six daily servings of coffee, tea or soda with more reflux than zero, so quantity appears to matter more than the drink itself. Reintroduce it alone and watch what happens.

Should I have had a bypass instead of a sleeve if I already had heartburn?

For many patients with real preoperative reflux, yes. A 2025 multicenter study of 672 patients found reflux symptoms before surgery nearly tripled the odds of reflux afterward. An eight-year comparison found confirmed GERD in 94.7% of sleeve patients against 25.0% of bypass patients. That gap is why preoperative testing should drive the choice of operation.

An endoscopy cannot tell you if you have reflux. It can only tell you if you have complications of GERD.
