The 4-Hour Test, and the Mistake That Wastes It
Gastroparesis is confirmed with a scintigraphic gastric emptying study run over four hours. You eat a light meal containing a small amount of radioactive material, usually eggs and toast or oatmeal. A scanner tracks how much of it is still sitting in your stomach. With an egg-based meal, more than 10 percent retained at the four-hour mark is the threshold most centers use for delayed emptying. Studies cut short at 90 minutes or two hours miss cases.
Now the part that catches people out.
Several drug classes distort the result. The 2022 ACG clinical guideline on gastroparesis directs patients to stop pro-motility agents, anti-nausea medications, opioids, marijuana, and neuromodulators such as nortriptyline before the study. Take any of them the day before and the number you get back describes the drug, not your stomach.
For a Clearwater patient that is not a small administrative detail. It is a four-hour appointment, a fasted morning, and two trips across the bay, repeated from the beginning. Ask for the stop list in writing when you schedule, and confirm it against every prescription you take, including the ones prescribed by someone else.
An upper endoscopy is not a test for gastroparesis. It is a test for everything gastroparesis pretends to be. It rules out an actual blockage. It also produces one useful clue by accident: if there is still food in your stomach after a long overnight fast, that points toward delayed emptying before any scan is run. Why Medication Stops Being the Answer
Metoclopramide, sold as Reglan, is the only medication the FDA has approved for gastroparesis in the United States. Prescribing is capped at three months because of the risk of serious neurological side effects with longer use.
That single fact explains a pattern most patients live through without anyone naming it. You get diagnosed. You start Reglan. It helps. The clock runs out. You are back where you started, and now you are being told to consider a procedure, which feels like it came out of nowhere. It did not. The ceiling was built into the prescription.
The alternatives each have a limit. Erythromycin works, then stops working as your body adapts, and it can cause diarrhea. Domperidone has a gentler side effect profile but is difficult to obtain in the US. Zofran, Compazine, and Benadryl treat nausea and vomiting, which is worth doing, but none of them speed up your stomach.
Diet is not a consolation prize here. Small frequent meals, thoroughly chewed and well-cooked food, low fat, and cutting carbonation, alcohol, and smoking produce real improvement for many patients. Keeping your calories and nutrition up is the first goal of treatment, not the last resort. A referral to a dietitian who has actually managed gastroparesis is worth asking for. The Procedures, Ordered by How Far They Go
Every surgical option for gastroparesis targets the same muscle. The pylorus is the valve between your stomach and small intestine. Loosening it lets the stomach drain. What separates the procedures is how much of your anatomy gets changed to do it.

Making the Trip from Clearwater
Two routes work from Pinellas County. The Courtney Campbell Causeway carries State Road 60 for 9.9 miles straight across Old Tampa Bay and drops you into Tampa near Rocky Point, which is the direct line to South Howard Avenue. The Howard Frankland Bridge on I-275 is the alternative, though it is in the middle of a multi-year expansion. Off-peak, either one runs about 35 minutes from central Clearwater.

Fill out your patient forms before you leave Clearwater rather than in the waiting room. Bring your prescription list, any prior gastric emptying study results with the actual four-hour percentages, and the report from any upper endoscopy you have had.

You have probably been told your test was fine, or that this is anxiety, or that you should try another round of Reglan. If your four-hour study has never been done, or was cut short, or was run while you were still taking something that skews it, you do not yet have an answer.

An endoscopy cannot tell you if you have reflux. It can only tell you if you have complications of GERD.
