There is no single best reflux operation. Recovery, and what the long-term data really says

Most laparoscopic and robotic hiatal hernia repairs are outpatient. You go home the same day, you’re back to light work in about a week and a half, and you’re eating normally around four to six weeks as your diet steps up from liquids to soft foods to regular over two to three months. Heavy lifting waits about six weeks. Some bloating in the first couple of weeks is normal while your stomach adjusts to the wrap. In appropriately selected patients, the practice reports that more than 90 percent get lasting relief and stop their daily reflux medication.
Now the honest part about durability, because most pages skip it. On imaging, a hiatal hernia comes back over the long term in roughly 3 out of 10 people. A 13-year randomized trial in JAMA Surgery found radiologic recurrence in 31 percent of suture repairs and 38 percent with mesh, and a pooled analysis put both near 31 percent. The reassuring part: most of those recurrences are small, reflux stayed controlled, and only about 6 to 8 percent ever needed a second operation. A separate long-term follow-up from a specialist center found the same pattern, recurrence common on imaging but usually without symptoms, and under 10 percent needing redo surgery.
That same trial found mesh did not lower the long-term recurrence rate and left more people with trouble swallowing solids. So for most standard repairs, Dr. Grandhige closes the diaphragm with sutures rather than reaching for mesh by default. Realistic expectation matters here: surgery fixes reflux, not every symptom, and most people still watch portions and a few triggers afterward.
