What gastroparesis is, and how it differs from reflux
Gastroparesis means your stomach empties too slowly even though nothing is blocking it. The muscles that normally grind food and push it into the small intestine work weakly or not at all, so food sits too long. That delay causes the familiar symptoms: feeling full after a few bites, nausea, bloating, and sometimes vomiting food hours after a meal. It isn’t common. According to the National Institute of Diabetes and Digestive and Kidney Diseases, roughly 10 men and 40 women out of every 100,000 have it. Reflux and gastroparesis are different problems that often show up together. Reflux (GERD) is stomach contents moving up into the esophagus. Gastroparesis is the stomach failing to move contents down. When the stomach stays full longer than it should, pressure builds and reflux can get worse, which is why bloating, heartburn, and regurgitation so often overlap. Sorting out which symptom comes from which problem is the first job of a real evaluation, and it’s why Dr. Grandhige treats GERD and hiatal hernias in the same practice as gastroparesis.
How gastroparesis is diagnosed
Gastroparesis can’t be diagnosed from symptoms alone, and that’s where a lot of patients get stuck. The test that confirms it is a four-hour gastric emptying study. You eat a standard meal that contains a small, safe amount of tracer, then a scanner measures how much food is still in your stomach over four hours. In general, delayed emptying is diagnosed when more than 10 percent of the meal is still there at the four-hour mark. The four-hour length matters: shorter studies miss cases, so a two-hour scan alone can read as normal when the problem is real. Dr. Grandhige’s standard is a proper four-hour study before anyone talks about a procedure.
An upper endoscopy is usually done first to rule out a blockage, but a normal endoscopy does not rule out gastroparesis. Endoscopy looks for damage and obstruction; it does not measure how fast the stomach empties. When reflux symptoms are part of the picture, Dr. Grandhige adds pH and impedance testing, and when swallowing or motility is in question, esophageal manometry to rule out disorders like achalasia that can copy these symptoms but need completely different treatment.
