Testing comes first, every time
Dr. Grandhige confirms reflux with objective testing before he recommends surgery, because a normal endoscopy does not rule reflux out and symptoms alone are unreliable.
Upper endoscopy shows damage, such as inflammation, Barrett’s esophagus, or a visible hernia, but it does not measure reflux or how the sphincter works. Plenty of people with real reflux have a completely normal endoscopy, especially while taking medication. pH monitoring is the test that actually measures reflux: how often it happens, how long each episode lasts, how acidic it is, and whether it lines up with your symptoms. Esophageal manometry measures how well your esophagus pushes food down, which decides whether a procedure is safe for you and whether a full or partial wrap is appropriate. Skipping manometry is one of the most common reasons people have trouble swallowing after reflux surgery.
For throat symptoms like chronic cough, hoarseness, and constant throat clearing, Dr. Grandhige uses a customized 24-hour pH-impedance probe that measures reflux at two levels, above the lower esophageal sphincter and above the upper esophageal sphincter. Standard testing usually checks only the lower level. This matters because the throat and voice box can react to a single reflux episode a day, while the esophagus can tolerate 40 to 50. Careful two-level testing raises the odds that surgery helps silent reflux (LPR) patients to roughly 80%, compared with about 50% when patients are chosen on symptoms alone.
Your surgical options, compared
There is no single best reflux operation. The right one depends on your anatomy, how well your esophagus works, your symptoms, and your goals. Because Dr. Grandhige performs all of them, the choice is driven by what fits you, not by the one procedure a surgeon happens to offer.
What recovery looks like
Most reflux procedures here are outpatient. You go home the same day, walk and drink liquids soon after surgery, and recover at home.
Fundoplication and LINX take about 1.5 to 2 hours under general anesthesia, done robotically or laparoscopically through a few small incisions. TIF takes about 45 to 60 minutes through the mouth. A few patients stay overnight for observation. Your diet moves from liquids to soft foods to regular textures over the following weeks, and full activity, including heavy lifting, comes back around 2 to 3 months. Some early trouble swallowing is normal and settles as the swelling goes down.
One instruction matters more than it sounds: avoid vomiting and retching in the early weeks, because it can strain a fresh hiatal hernia repair. Most well-selected patients with typical reflux come off their daily medication and stay off it.
Why patients cross the bay for this practice
Reflux surgery succeeds or fails on two things, an accurate diagnosis and the right procedure choice, more than on the operation itself. This practice is built around both.
Dr. Grandhige works only on the esophagus, diaphragm, and stomach. He has completed more than 600 fundoplications, more than 600 LINX procedures, and more than 200 TIF cases, and he is the only board-certified surgeon in the Tampa Bay area performing all three regularly. He earned his medical degree at the University of Michigan, trained in general surgery at Yale-New Haven Hospital, and completed a fellowship there in foregut and minimally invasive surgery. He is a Fellow of the American College of Surgeons, a member of SAGES, and a Founding Member of the American Foregut Society, and he has practiced in the Tampa Bay area since 2009. You can read his full background on the about page.
Every operation is performed at HCA South Tampa Hospital with the same operating room team, and a dedicated physician assistant is in every case and stays involved through your recovery. When Dr. Grandhige is not immediately reachable, that same PA already knows your anatomy and your procedure, so you are never handed to a provider who does not know your case.
For St. Petersburg patients, the office is about 25 minutes away across the Gandy Bridge. When you live across the water, the practice coordinates as much of your testing as possible closer to home and consolidates your visits, so you make fewer trips over the bridge.

Getting seen from St. Petersburg
Tampa Bay Reflux Institute is at 1315 South Howard Avenue, Suite 101, in South Tampa, about 25 minutes from St. Petersburg across the Gandy Bridge.
The office sits in a yellow brick building next to Sally O’Neal’s Pizza, with parking behind the restaurant. Office hours are 9 a.m. to 6 p.m., Monday through Friday, and you can reach the office at 813.922.2920.
Before your visit, email your prior records to info@tampareflux.com: endoscopy and pathology reports, any pH or manometry results, imaging, and notes from your gastroenterologist, ENT, or primary care doctor. Sending records ahead turns your first visit into a real diagnostic conversation instead of paperwork. Most patients are seen within two weeks and always within four. If your testing is already done, surgery can often be scheduled about four weeks after the first visit. If you still need testing, plan on roughly eight weeks from first visit to surgery. St. Petersburg, Clearwater, and the rest of Pinellas are a regular part of the practice, and the team is used to coordinating care for patients coming from across the bay.
