Best Thing For Heartburn Relief In St. Petersburg, FL

If antacids and daily pills keep letting your heartburn back, the reason is usually mechanical, not too much acid. Dr. Gopal Grandhige tests to find the exact cause, then matches the right treatment to your anatomy. He treats reflux and nothing else, at a South Tampa office a short drive across the bay from St. Petersburg.

Why your heartburn keeps coming back

Heartburn keeps coming back because most reflux is a mechanical problem, and acid-reducing pills don’t repair the mechanics. The valve at the top of your stomach, the lower esophageal sphincter, gets weak. Often a hiatal hernia pulls part of the stomach up through the diaphragm and breaks the natural barrier. Antacids, H2 blockers, and proton pump inhibitors lower or neutralize acid, but the valve still leaks. So reflux continues, now carrying bile and digestive enzymes along with less acid, and the burn returns the moment a dose wears off.

St. Petersburg living doesn’t help. Late dinners on the patio, fried or rich seafood, a couple of drinks, and carbonation all raise pressure in the stomach and relax that valve, especially when you lie down within a few hours of eating. Reflux and GERD are common, and not everyone with reflux even gets classic heartburn. That is why the same symptom can have very different causes, and why guessing at treatment so often fails.

How Dr. Grandhige finds the cause before recommending anything

Illustration of the esophagus and stomach showing the muscular valve, supporting diaphragm, and the angle that work together to hold back reflux

Before any treatment, Dr. Grandhige proves whether reflux is happening, why, and whether it is even the source of your symptoms. He uses objective testing, not a trial of pills. His rule is simple: the decision is the hard part, and the operation is the last step.

Four tests answer four different questions. An upper endoscopy shows damage like inflammation or Barrett’s esophagus, but it does not diagnose reflux, and a normal scope does not rule it out. Esophageal pH monitoring, worn over 48 to 72 hours during your normal day, measures how often reflux happens, how long it lasts, and whether it lines up with your symptoms. According to the National Institute of Diabetes and Digestive and Kidney Diseases, pH and impedance monitoring is the most accurate way to detect acid reflux. Esophageal manometry measures the strength and coordination of your esophagus, which decides what procedure is safe, because a weak esophagus rules out some operations. A barium swallow adds a moving picture of how you actually swallow when it is needed. Skip these, and surgery gets done on the wrong diagnosis, which is how reflux surgery earned a bad name in the first place.

Silent reflux (LPR): the reflux most tests miss

If your main problems are a chronic cough, throat clearing, hoarseness, or a lump-in-the-throat feeling, standard reflux tests can miss the cause, because they don’t measure reflux high enough. This is silent reflux, or LPR, where stomach contents reach the throat and voice box. Many people with LPR have little or no heartburn, so they get treated for allergies, asthma, or sinus trouble for years while the real cause is never measured.

The tissues involved explain why. The esophagus can shrug off 40 to 50 reflux episodes a day, but the throat and voice box can react to as little as one a day. Dr. Grandhige uses a customized 24-hour dual-channel pH-impedance probe that measures reflux above both the lower and the upper esophageal sphincter, and it detects non-acid reflux, like bile and pepsin, that acid-only tests never see. The payoff is patient selection. Without this testing, surgery for LPR is often quoted around a 50 percent chance of helping. With it, Dr. Grandhige identifies the patients who will actually benefit and raises that closer to 80 percent. He operates on fewer people as a result, and on the right ones.

Your four procedure options, matched to your anatomy

There is no single best reflux operation. The right one depends on your anatomy, esophageal strength, hernia size, and goals, and Dr. Grandhige performs all four so the choice fits you rather than his habit. He has done over 600 fundoplications, over 600 LINX procedures, and over 200 TIF procedures, and he is the only board-certified surgeon in the Tampa Bay area performing all of them regularly. In 2026 he is adding RefluxStop, which will make him the only surgeon in the area offering all four anti-reflux procedures.

Procedure

How it works

Often a fit for

Burping and vomiting

Repairs a hiatal hernia

Fundoplication (Nissen, Toupet, Dor, Watson) Wraps the top of the stomach around the lower esophagus to rebuild the valve, done robotically or laparoscopically Larger hernias, weaker esophageal motility (partial wraps), and patients who want the most durable, longest-studied repair May be limited, depending on the wrap Yes
LINX A ring of magnetic titanium beads that supports the valve and opens as you swallow Good esophageal motility, patients who want to keep burping and vomiting, and those who want a reversible option; works with MRI up to 1.5 Tesla Usually preserved once early swelling settles Yes, repaired at the same time
TIF (EsophyX) Rebuilds the valve from inside, through the mouth, with no incisions Little or no hiatal hernia, mild to moderate reflux, no Barrett’s or severe esophagitis, and not obese Usually preserved No, it does not address the diaphragm
RefluxStop Newest anti-reflux option, being added in 2026 Decided case by case after testing See the procedure page See the procedure page

Column 1 Value Column 2 Value

Two practical notes. Fundoplication and LINX each take about 1.5 to 2 hours, and TIF runs about 45 to 60 minutes. Most patients go home the same day and move through a staged diet over a few weeks to a couple of months. Dr. Grandhige is direct about the tradeoff with TIF: it is the strongest incisionless option, but it does not repair a hiatal hernia and it is not permanent, with a failure rate he quotes at about 2 percent a year.

What relief actually looks like, and what it won’t fix

When reflux is proven and the right procedure is matched to the right patient, results are strong. For patients with typical symptoms, heartburn and regurgitation, who have objective reflux, suitable anatomy, and good esophageal strength, Dr. Grandhige reports that more than 95 percent get significant relief and stop taking daily reflux medication. For silent reflux, the dual-channel testing above raises the odds of improvement to roughly 80 percent.

Honesty is part of the plan. No operation fixes every symptom. Problems like esophageal hypersensitivity, motility disorders, and functional chest pain can mimic reflux but do not improve with reflux surgery, and Dr. Grandhige turns those patients away rather than operate. Typical reflux symptoms can settle quickly, while LPR symptoms often take four to six months. Patients and referring physicians tend to say the same things: the evaluation was thorough, the reasoning was explained in plain language with hand-drawn diagrams, and no one was pushed toward surgery.

Seeing Dr. Grandhige from St. Petersburg

Reflux surgery outcomes depend more on diagnosis and procedure selection than on the operation itself, and Dr. Grandhige has focused only on foregut surgery since 2009. He is a board-certified general surgeon, a Fellow of the American College of Surgeons, a member of SAGES, and a founding member of the American Foregut Society. He trained at Johns Hopkins, earned his MD at the University of Michigan, and completed his residency and fellowships in foregut and minimally invasive surgery at Yale-New Haven. You can confirm his board certification directly through the American Board of Surgery. A dedicated physician assistant is in every one of his operations, his office staff have been with him for years, and he is reachable after hours.

St. Petersburg patients see him at the South Tampa office, a short drive over the Gandy or Howard Frankland Bridge. Most people are seen within two weeks and always within four. If your testing is already done, one visit is often enough. If not, expect two visits about four weeks apart, and roughly four to eight weeks from decision to surgery. The team coordinates testing so out-of-town patients make as few trips as possible, and sending your records to the office before your visit means the consult is about answers, not paperwork. Come in if you have a hiatal hernia over 3 centimeters, symptoms breaking through daily medication, a need for more than once-daily pills, complications like severe esophagitis or Barrett’s esophagus, or if you would rather not stay on acid-reducing medication for life.

FAQS

For quick, temporary relief, an antacid neutralizes stomach acid within minutes, and an H2 blocker starts working in about half an hour and lasts several hours. Both ease the symptom. Neither repairs the weak valve behind most reflux, so if heartburn keeps returning, the fast fix is not the real fix.

Medication and lifestyle changes manage symptoms by lowering acid and reducing triggers. When the cause is a weak valve or a hiatal hernia, only a mechanical repair addresses the root problem. Testing shows which situation you are in and whether a repair is worth considering.

Lying down removes gravity’s help, and a hiatal hernia makes nighttime reflux more likely, so stomach contents reach the esophagus and throat more easily during sleep. Eating within three hours of bed makes it worse. Raising the head of the bed and finishing meals earlier both reduce nighttime episodes.

No. Surgery is one option, not the default. Many people do well on medication and lifestyle changes, and Dr. Grandhige recommends a procedure only when testing shows a mechanical problem it can fix. Many patients leave the consultation with a non-surgical plan.

GERD mostly affects the esophagus and usually causes heartburn or regurgitation. LPR is reflux that reaches the throat and voice box, often showing up as cough, hoarseness, or throat clearing with little or no heartburn. LPR needs testing that measures reflux higher up, which standard reflux tests skip.

It depends on the procedure. A full fundoplication can limit both. LINX and TIF usually preserve them once early swelling settles. This is one of the main reasons the right procedure is matched to the right patient.

Most patients are seen within two weeks and always within four. If your testing is complete, one visit is often enough. If not, plan on two visits about four weeks apart, and roughly four to eight weeks from decision to surgery.

Stop guessing why the burn keeps coming back

You do not have to stay on medication that only half works, or accept heartburn as permanent. Objective testing can tell you whether reflux is really the cause, why it is happening, and what will actually help, whether that turns out to be medication, a procedure, or neither.
