Best Surgeon For Hiatal Hernia Repair In St. Petersburg, FL
The right hiatal hernia repair starts with proving reflux, not assuming it. Dr. Gopal Grandhige is a board-certified surgeon whose practice treats only foregut disease, the esophagus, diaphragm, and stomach. He repairs hiatal hernias for St. Petersburg patients about 25 minutes away in South Tampa, and he is the only board-certified surgeon in the Tampa Bay area who regularly performs all four anti-reflux procedures: fundoplication, LINX, TIF, and RefluxStop. Many St. Pete patients make the short drive for one reason: a surgeon who does this every day, not a few times a year.

A Hiatal Hernia Is a Mechanical Problem, So Medication Cannot Fix It
A hiatal hernia happens when the opening in your diaphragm where the esophagus passes through stretches or weakens, letting the stomach slip upward into the chest. That breaks the partnership between two structures that normally work together to stop reflux: the lower esophageal sphincter and the diaphragm. Roughly 90% of people with reflux disease have a hiatal hernia, even a small one that an endoscopy can miss.
Acid-suppressing medication reduces how acidic your reflux is. It does not strengthen a weak sphincter, repair the diaphragm, or stop reflux events from happening. So reflux of bile, pepsin, and other digestive enzymes continues, and tissue damage can keep occurring even when heartburn feels better. That is why a patient can take a proton pump inhibitor twice a day for years and still wake up at night choking. The mechanical defect was never addressed. Repairing the hernia is the part that fixes the mechanics.
Objective Testing Comes Before Any Surgical Decision
Reflux symptoms are not proof of reflux. Esophageal hypersensitivity, functional chest pain, motility disorders, and ENT or pulmonary conditions all mimic it. Operating on the wrong diagnosis is the single biggest reason reflux surgery developed a poor reputation decades ago. Dr. Grandhige’s evaluation is built to prevent exactly that, using three tests that each answer a different question.
Upper endoscopy looks at anatomy and damage. It shows esophagitis, Barrett’s esophagus, strictures, and hiatal hernia, but it does not diagnose GERD. A normal endoscopy does not rule reflux out. Many patients with significant reflux have a completely normal scope, especially if they are on medication.
pH monitoring is the test that actually proves reflux. A wireless Bravo capsule or a catheter-based probe records how often reflux happens, how long episodes last, how acidic they are, and whether your symptoms line up with reflux events, over 48 to 72 hours of normal daily life. This is what separates “you probably have reflux” from “your reflux is documented.”
Esophageal manometry measures whether the esophagus has the strength and coordination to handle a repair. This is the most overlooked test, and the most consequential. Weak motility changes everything: it determines whether a full or partial wrap is safe, whether LINX is appropriate, or whether surgery should be avoided. Skip manometry and you risk leaving a patient unable to swallow comfortably.
For throat-dominant symptoms, the testing goes a step further, which is covered in the section on silent reflux below. The principle is consistent: prove the problem, then fix it.
Choosing the Procedure: There Is No Single “Best” Repair
The right operation depends on your anatomy, your esophageal function, your symptoms, and your goals. Because Dr. Grandhige performs four anti-reflux procedures, the recommendation is driven by what fits your body, not by the one operation a surgeon happens to be comfortable with. Here is how the main options differ.
A fundoplication uses the upper part of your own stomach, wrapped around the lower esophagus, to rebuild the reflux barrier. It comes in several configurations (Nissen, Toupet, Dor, Watson), it is very durable, it has decades of outcome data, and it can address large hernias and weaker motility by tailoring the wrap. The trade-off: depending on the wrap, it can limit the ability to burp or vomit and may cause more gas-bloat.
The LINX device is a ring of magnetic titanium beads placed around the lower esophagus. It does not wrap the stomach, so it preserves more normal anatomy, usually keeps the ability to burp and vomit, and tends to cause less long-term bloating. It opens dynamically when you swallow and resists reflux when stomach pressure rises. It requires good esophageal motility, and it carries a somewhat higher risk of early swallowing difficulty. Current devices are MRI-compatible up to 1.5 Tesla, and LINX can be removed if needed.
TIF (transoral incisionless fundoplication) is performed entirely through the mouth with no incisions, using the EsophyX device to fold the stomach around the lower esophagus from the inside. It is the best endoscopic option that exists, but it is for a narrow group: minimal or no hiatal hernia, mild to moderate reflux, no severe esophagitis or Barrett’s, and not obese. Because it cannot repair the diaphragm, it is not a substitute for surgery in a patient who needs the hernia fixed. Dr. Grandhige quotes a failure rate of about 2% per year and frames TIF as a durable solution that works for many years rather than a permanent one.
RefluxStop is the fourth option Dr. Grandhige is adding, making this the only Tampa Bay practice offering all four. The point of carrying every option is restraint: when surgery is the answer, the procedure is matched to your physiology, and when it is not the answer, you are told so.
If Your Symptoms Are in Your Throat: Silent Reflux Changes the Testing
If your main symptoms are hoarseness, chronic throat clearing, cough, or a lump-in-the-throat feeling, standard reflux testing often misses the cause. This is laryngopharyngeal reflux (LPR), or silent reflux, and it behaves differently from classic GERD. Only about a third of suspected LPR patients have typical heartburn, according to the American Academy of Otolaryngology-Head and Neck Surgery. The throat and voice box are far more sensitive than the esophagus: the esophagus may tolerate 40 to 50 reflux episodes a day, while the larynx can develop symptoms from a single daily episode.
Standard testing only measures reflux above the lower esophageal sphincter, so it cannot tell whether reflux is actually reaching the throat. Dr. Grandhige uses a customized 24-hour pH-impedance dual-channel probe that measures reflux at two levels, above the lower sphincter and above the upper sphincter, and measures non-acid reflux like bile and pepsin that standard acid testing cannot detect. The clinical payoff is concrete: without this precision, LPR patients are routinely quoted a roughly 50% chance that surgery helps. With proper dual-channel testing and patient selection, symptom improvement rises to approximately 80%. That means operating on fewer patients, but the right ones. You can read more about silent reflux and how it differs from typical GERD. One honest expectation: LPR symptoms typically take four to six months to improve, while classic heartburn often resolves almost immediately.
Why Volume and a Consistent Team Change the Result
All of Dr. Grandhige’s surgeries are performed at HCA Florida South Tampa Hospital, deliberately, at a single facility with the same operating room team and anesthesiologists. Foregut surgery is technically nuanced: small differences in positioning, instrument choice, and repair tension affect swallowing and durability years later. A team that does these procedures routinely anticipates the steps and catches early warning signs. A dedicated physician assistant assists in every case and is directly reachable for questions, and the office medical assistants have been with the practice over a decade, so patients are never handed to an unfamiliar provider. For background on the surgeon and team, see the About the practice page.
This is the difference between choosing an individual and choosing a system. A surgeon operating occasionally across several hospitals with rotating staff cannot offer the same consistency as one who works daily inside a refined, predictable setup.
Credentials You Can Verify
Dr. Grandhige earned his biology degree at Johns Hopkins University and his medical degree at the University of Michigan, then completed his general surgery residency and fellowships in foregut and minimally invasive surgery at Yale-New Haven Hospital. He is a Fellow of the American College of Surgeons, a member of the Society of American Gastrointestinal and Endoscopic Surgeons, and a Founding Member of the American Foregut Society. He has focused on reflux and foregut disease since 2009, more than 16 years in the Tampa Bay area.
Credentials are necessary, but every qualified surgeon has them, so they are not what sets a practice apart. What differentiates this one is judgment: objective testing before decisions, conservative recommendations when appropriate, and a documented willingness to say no to surgery. Patients can independently confirm board certification through the American Board of Surgery and active licensure through the Florida Department of Health.
