The best heartburn relief starts with proof, not a prescription
The relief that lasts comes from proving reflux before treating it, not from escalating medication or operating on symptoms alone. Dr. Grandhige confirms reflux with objective testing, then matches treatment to what the testing shows.
Two tests do most of the work. Esophageal pH monitoring records how often reflux happens, how long it lasts, and whether it lines up with your symptoms, usually over 48 to 72 hours while you eat, sleep, and go about a normal day. Esophageal manometry measures the strength and coordination of the esophagus, which decides whether a given procedure is safe or would leave you unable to swallow comfortably. Skipping these tests is how patients end up with the wrong operation and lasting difficulty swallowing.
Testing also protects you from surgery you do not need. Heartburn-like symptoms can come from esophageal hypersensitivity, a motility disorder, or a functional condition that surgery will not fix. Because every recommendation here starts with data, Dr. Grandhige regularly tells patients that their symptoms are not mechanical reflux and that an operation would not help. The four anti-reflux procedures, and how the right one is chosen
There is no single best reflux operation. The right one depends on your anatomy, how well your esophagus moves food, the size of any hiatal hernia, and what matters most to you, which is exactly why it helps to see a surgeon who performs more than one.
Silent reflux (LPR): when the real problem is in your throat

If you have a chronic cough, constant throat clearing, hoarseness, or a lump-in-the-throat feeling, and your endoscopy came back normal, reflux reaching your throat may be the cause even without classic heartburn. This is laryngopharyngeal reflux, often called silent reflux or LPR.
The throat and voice box are far more sensitive than the esophagus. The esophagus can tolerate dozens of reflux episodes a day, while the larynx can react to as little as one. That is why LPR is so often mistaken for allergies, asthma, or sinus disease, and why standard reflux testing, which measures acid only low in the esophagus, misses it.
Dr. Grandhige uses a customized 24-hour dual-channel pH-impedance probe that measures reflux above both the lower and the upper esophageal sphincters and detects non-acid reflux such as bile and pepsin, which standard acid testing cannot see. That precision changes outcomes. Patients with throat symptoms are commonly quoted around a 50% chance that surgery helps; by confirming that reflux actually reaches the throat before recommending anything, the practice reports improvement closer to 80% in the patients it selects. The result is fewer operations, done on the right people. LPR symptoms also improve more slowly than heartburn, usually over four to six months, and Dr. Grandhige sets that expectation up front.
Traveling from Clearwater: how care works
Tampa Bay Reflux Institute sits at 1315 South Howard Avenue, Suite 101, in South Tampa, about a 30-minute drive from Clearwater. Most patients are seen within two weeks and always within four. If you already have testing done, one visit is often enough to confirm the diagnosis and discuss options. If testing is still needed, the first visit covers review and planning, and a second visit about four weeks later goes over results and the plan, with surgery typically four to eight weeks out when it is appropriate.

Dr. Gopal Grandhige, MD, is a board-certified general surgeon who has focused only on reflux and foregut disease since 2009. He founded Tampa Bay Reflux Center in 2009 and continues that work today as founder and medical director of Tampa Bay Reflux Institute.
