Yes, vaping can make reflux worse, and the reason isn’t complicated. Nicotine relaxes the ring of muscle that keeps stomach acid out of your esophagus. I’ve lost count of the patients who quit cigarettes, switched to a vape, and came back six months later confused about why their heartburn never improved.

Vaping and GERD are connected through nicotine. Nicotine lowers pressure in the lower esophageal sphincter, the valve between your stomach and your esophagus. When that pressure drops, acid travels the wrong way. Regular vapers show more daily reflux episodes and lower esophageal pH than people who don’t vape.

Most articles on this subject hedge everything. I won’t.

What’s actually in a vape

A vape heats liquid into an aerosol you breathe in. That liquid usually carries nicotine, propylene glycol, vegetable glycerin, and flavoring chemicals.

Shapes vary. Pens, USB sticks, small boxes. Your esophagus doesn’t care about the shape.

CDC data put adult vaping at roughly 7% in 2024, nearly double the 3.7% recorded in 2020, and it held near that level through 2025.

GERD or acid reflux? Knowing the difference matters

Acid reflux is an event. GERD is a disease. The American College of Gastroenterology sets the line at symptoms two or more times a week, or any esophageal damage regardless of how often you feel it.

Why does that matter here? Because most vaping research measures reflux episodes, not GERD diagnoses. Those are two different claims. Anyone telling you vaping definitely causes chronic reflux disease is running ahead of the evidence.

Heartburn after a huge meal is normal. Burning four nights a week for two years isn’t.

Does vaping cause GERD?

Vaping raises the number of reflux episodes you have. Whether it produces full GERD is less settled, and your answer shifts depending on which study you read.

The strongest data landed in early 2025. Researchers publishing in Kazan Medical Journal ran 24-hour pH-impedance testing on 90 people, split into three groups of 30. Regular vapers averaged 54 acidic reflux episodes a day. Non-vapers averaged 21.5. Close to a third of the vapers crossed 80 episodes daily, well into pathologic territory.

A December 2024 Cureus paper surveyed 403 university students in Riyadh. Symptom scores climbed with frequency, and people vaping four to seven days a week scored worst. A 2025 Jazan study found GERD in 56.8% of e-cigarette users against 32.1% of non-users.

Now the other side. A 2023 Cureus study of 397 students in Jeddah found no statistically significant link once body weight was accounted for. Short exposure windows and questionnaires instead of real testing probably explain the gap, but it’s honest data and it belongs here.

Underneath all of it sits nicotine. Smokers carry lower valve pressure than non-smokers, and research in Gut found most of their reflux happened during deep breaths and coughing, when belly pressure spikes past what a weak valve can hold. Vaping means long, deep pulls. Forty times a day, that’s the same physics.

Nicotine source What it does to the valve Added irritation
Cigarettes Pressure drops sharply during use Smoke, cough, less saliva
Nicotine vapes Same nicotine effect, all-day use Warm aerosol, flavoring chemicals
Pouches, gum, lozenges Same nicotine effect Swallowed nicotine, evening use
Zero-nicotine vapes No nicotine effect Aerosol still hits raw tissue

What vaping does to your esophagus and throat

Vaping can inflame the esophagus directly, and it drives throat symptoms that look nothing like classic heartburn.

Case reports describe young, otherwise healthy vapers arriving with severe erosive esophagitis after heavy daily use. Some had mild, controlled reflux before they started. Most improved once they quit and began acid suppression.

Then the throat. Hoarseness, constant throat clearing, a cough that won’t quit. That pattern is silent reflux, and vapers take it from both directions, acid from below and aerosol from above. Patients blame the throat hit and stop investigating. Sometimes they’re right. Often they aren’t.

How do you get reflux under control?

Drop the nicotine first. Everything after this works better once you do.

  1. Cut nicotine in every form. Vapes, pouches, gum, lozenges. The valve doesn’t care how it arrived. Zero-nicotine liquid removes the main problem but still pushes warm aerosol past irritated tissue.
  2. Get objective testing before anyone calls you PPI-refractory. pH-impedance monitoring and manometry show what’s actually happening. At Tampa Bay Reflux Institute we ask every patient for their exact nicotine source and frequency. I’ve met people labeled refractory and steered toward an operation when nobody ever asked about the vape in their pocket.
  3. Fix the anatomy if it’s broken. A hiatal hernia lets acid through no matter how clean your habits become. Repair options include rebuilding the valve surgically, magnetic sphincter augmentation, or an incisionless repair performed through the mouth.

One more thing, and most practices skip it. Nicotine undermines how long a repair lasts. Operating on someone who’s still vaping sets up a failure nobody connects back to the real cause two years later.

What I tell patients who vape

If you vape and you have reflux, the vape is on the list. Maybe not the only item on it, but it’s the one you can remove today, for free. The link between vaping and GERD runs through nicotine, and nicotine doesn’t care whether it reached you as smoke, aerosol, or a pouch under your lip. Quit it, get tested properly, then decide what else needs repair.

FAQs

How strong is the evidence linking vaping and GERD?

Moderate and growing. The 2025 Kazan Medical Journal study using 24-hour pH-impedance testing found regular vapers averaging 54 acidic reflux episodes daily against 21.5 in non-vapers. Symptom studies agree, though a 2023 Jeddah paper found no significant link.

Will switching from cigarettes to vaping fix my acid reflux?

Usually not. Nicotine is the part that weakens your valve, and vapes still deliver it. You lose the smoke and the cough, which helps, but the valve stays loose. Many of my patients report no change after switching.

Does zero-nicotine vaping still cause reflux?

It removes the nicotine effect on the valve, which is the main driver. The aerosol can still irritate an already inflamed esophagus or throat, especially with silent reflux. Direct research on nicotine-free liquid is thin.

How soon after quitting vaping does reflux improve?

Case reports show symptoms easing within weeks to a few months, usually alongside acid suppression. Valve tone recovers, but no study has mapped the exact timeline. After years of reflux, expect improvement rather than a reset.

Can vaping and GERD damage your esophagus without heartburn?

Yes. Published cases describe severe erosive esophagitis in heavy vapers who never reported classic burning. Throat symptoms, trouble swallowing, or a lasting cough can be the only warning sign.

An endoscopy cannot tell you if you have reflux. It can only tell you if you have complications of GERD.
