If you want to know how to sleep with heartburn tonight, three moves do most of the work: roll onto your left side, raise your whole upper body 6 to 8 inches, and stop eating three hours before bed. In a 2022 study of 57 patients, acid cleared the esophagus in a median 35 seconds on the left side and 90 seconds on the right. Same patients, same night, opposite shoulder.

Nighttime heartburn is acid reflux that happens while you lie flat and asleep. Gravity stops holding stomach contents down, swallowing slows to almost nothing, and saliva output drops. Acid stays against the esophagus far longer than it does during the day, which is why nighttime reflux causes more damage than daytime reflux.

I repair reflux anatomy for a living, so I’ll say the quiet part first. These three moves are the best free tools you have, and for plenty of people they still won’t be enough.

Why Does Heartburn Get Worse at Night?

Lying flat removes gravity, your swallow rate collapses, and saliva stops buffering acid. Three defenses gone in the same hour.

Gravity quits the second you lie down

Upright, your stomach sits below your esophagus. Flat, acid gets a level runway instead of an uphill climb. If that valve is weak, or a hiatal hernia has pulled part of the stomach above the diaphragm, acid moves freely.

Fewer swallows, less saliva

Every swallow pushes refluxed acid back down, and saliva neutralizes the rest. During deep sleep you barely swallow and make far less saliva. So acid sits.

That’s why reflux does its worst work between midnight and 5 a.m.

Which Side Should You Sleep On for Heartburn?

Your left side. In that 2022 pH study, median acid exposure time was 0% on the left, 0.6% flat on the back, and 1.2% on the right.

The left side wins, and it isn’t close

Your stomach sits slightly left of your esophagus. Roll left and the junction between them ends up above the acid pool, so nothing waits at the valve. Clearance took 35 seconds on the left versus 76 on your back.

Forty seconds sounds trivial. Multiply it across 2,500 nights and you get a different esophagus at 55.

What’s so bad about the right side?

On the right, your stomach sits above the junction and acid pools at the valve. Clearance there took 90 seconds, the slowest of any position tested.

Switching feels wrong for about ten days. Wedge a body pillow against your back so you can’t roll. Most patients stop noticing.

How Should You Raise Your Upper Body?

Raise the head of the bed 6 to 8 inches so your torso sits on a ramp. In a crossover trial in Clinical Gastroenterology and Hepatology, a 10-inch wedge dropped supine acid exposure from 21% to 15%.

Why stacked pillows backfire

Pillows bend your neck and fold your abdomen, and abdominal pressure is one of the forces shoving acid upward in the first place. Worse problem, plus neck pain.

Actually, that framing isn’t quite right. Pillows aren’t useless, they’re aimed at the wrong body part. You want one continuous slope from hips to head.

Three ways to build the slope

  1. Risers or wood blocks under the head-end bed legs. Start here.
  2. A full-torso foam wedge, 6 to 8 inches at the high end.
  3. An adjustable base that lifts the head section.

Stack this with left-side sleeping. Together they beat either one alone.

What Should You Stop Eating Before Bed?

Anything, at least three hours before you lie down. Timing matters more than the specific food, which is the opposite of what most reflux articles tell you.

The trigger food list is weaker than you’ve been told

Chocolate, mint, citrus, tomatoes, spicy food, coffee. You’ve read that list a hundred times. The American College of Gastroenterology’s 2022 guideline does suggest avoiding trigger foods, but rates that as conditional, on low-quality evidence.

Lab work shows chocolate and peppermint lower pressure at the esophageal valve, which should mean symptoms. Observational studies failed to tie chocolate intake to actual reflux episodes. A 2020 review in the Cleveland Clinic Journal of Medicine found no consistent association between GERD and spicy food, coffee, chocolate, citrus, or mint.

Two things hold up better. Evening alcohol slows acid clearance while you’re flat. And a large cross-sectional study in Clinical Gastroenterology and Hepatology tied carbonated drinks to a 24% higher rate of heartburn that wakes people from sleep. The data fights with itself.

A blanket elimination diet wastes your evenings. That’s my opinion, and it isn’t popular. Log two weeks, find the two or three items that actually get you, and eat the rest.

Safer choices for a late snack

Small, low-fat, low-acid. Johns Hopkins points reflux patients toward high-fiber whole grains, non-citrus fruit, and nuts. A banana, a few almonds, plain oatmeal. Ginger or chamomile tea beats peppermint tea, which may relax the valve you’re protecting.

Evening Habits That Cut Nighttime Reflux in 2026

Your last meal is the biggest evening variable you control. In a matched case-control study, people who ate within three hours of bed had 7.45 times the odds of GERD compared with people who waited four hours or more.

Your last meal is the biggest lever

Eat at 6, sleep at 10. That’s the whole rule.

If a 9 p.m. dinner isn’t negotiable, make it your smallest meal and keep the fat low, since fat slows how fast your stomach empties.

Loose clothes, a short walk, a calm hour

Tight waistbands press on your abdomen and push stomach contents up, so sleep in something loose. A 15-minute walk after dinner moves food along. Skip hard exercise inside two hours of bed.

When Is Nighttime Heartburn Something Worse?

Heartburn two or more nights a week crosses from annoyance into gastroesophageal reflux disease. The 2025 ASGE guideline puts GERD in roughly a third of U.S. adults.

Heartburn two nights a week isn’t normal

If your symptoms are morning hoarseness, throat clearing, a lump in your throat, or a cough that only shows up at night, you may have silent reflux rather than classic heartburn. I’ve met patients who spent four years on allergy medication before anyone checked the valve.

Red flags that mean you stop waiting

  • Trouble swallowing, or food that feels stuck
  • Weight loss you didn’t plan
  • Vomiting blood, or black stools
  • Hoarseness or a cough that won’t quit
  • Chest pain, after ruling out your heart

Years of acid exposure can turn the esophageal lining into Barrett’s esophagus, which raises esophageal cancer risk. Roughly 10% to 15% of people with GERD develop it.

Do Drugstore Medicines Fix Nighttime Heartburn?

They control symptoms. They don’t repair the valve letting acid through. Fine for a bad night, a holding pattern for a bad decade.

What each one actually does

Option Starts working Lasts Best night use
Antacids Minutes 1 to 2 hours You’re already awake and burning
H2 blockers 1 to 3 hours 10 to 12 hours 30 to 60 minutes before bed
PPIs Days to full effect About 24 hours Daily control, taken before a meal

H2 blockers come with a catch. Take one every single night and tolerance builds, often inside two weeks. The drug fades, people double the dose, nothing improves. Use it on the nights you need it.

PPIs are stronger and standard for daily control, but 30% to 40% of people still have symptoms on them. If that’s you after eight weeks, more medication isn’t the answer. Testing is.

When Learning How to Sleep With Heartburn Isn’t Enough

Give the position changes four to six weeks. If you’re still waking with acid in your throat after doing all of it correctly, the problem isn’t your bedtime routine. It’s your anatomy.

I’ve watched this play out dozens of times. Someone does everything right for three years, stays on a PPI the whole time, then turns up with a 4 cm hiatal hernia no wedge pillow was ever going to fix. Testing sorts it out: pH monitoring, manometry, and an endoscopy that measures the hernia and the valve.

From there the options are real. An incisionless repair done through the mouth works well for small hernias. A laparoscopic repair handles larger ones and severe disease. Which fits depends on hernia size and esophageal motion, and recommending a procedure before measuring both is guesswork.

Knowing how to sleep with heartburn should buy you quiet nights, not a lifelong workaround for a broken valve. FAQs

What is the best way to sleep with heartburn?

Lie on your left side with the head of your bed raised 6 to 8 inches. A 2022 study of 57 patients found acid cleared the esophagus in a median 35 seconds on the left side, compared with 90 seconds on the right. Add a three-hour gap between your last meal and bedtime.

Why is right-side sleeping worse for acid reflux?

On your right side, the stomach sits above its junction with the esophagus, so acid pools directly at the valve. Researchers measured 90 seconds of acid clearance time on the right side, the slowest of any sleep position tested.

Is a wedge pillow enough, or should I raise the whole bed?

Either works, as long as it lifts your entire torso rather than just your head. A crossover trial found a 10-inch wedge lowered supine acid exposure time from 21% to 15%. Stacking regular pillows doesn’t count, because it folds the abdomen and can make reflux worse.

How long before bed should I stop eating?

At least three hours. A matched case-control study found that people with a dinner-to-bed gap under three hours had 7.45 times the odds of GERD compared with those who waited four hours or more. Four hours is better if your schedule allows it.

Can I take famotidine every night for nighttime heartburn?

Nightly use tends to lose its effect, since tolerance develops within roughly one to two weeks. Famotidine works best taken 30 to 60 minutes before bed on the nights you need it. Needing it every night for more than a couple of weeks is a signal to get tested, not to raise the dose.

When does knowing how to sleep with heartburn stop being enough?

If left-side sleeping, bed elevation, and a three-hour eating cutoff haven’t helped after four to six weeks, the cause is likely structural. A hiatal hernia or a failed valve won’t respond to position changes. Between 30% and 40% of people on acid-suppressing medication still have symptoms, and that group needs testing rather than more pills.

An endoscopy cannot tell you if you have reflux. It can only tell you if you have complications of GERD.
