Acid reflux at night is stomach acid backing up into your esophagus while you lie down. It burns worse than daytime reflux for one reason. When you sleep, you swallow less and make less saliva, so acid sits against the esophageal lining far longer before it clears.
Three things fail at once. Gravity stops helping. Your stomach empties slower. And the muscle ring at the bottom of your esophagus relaxes on a schedule you don’t control.
Up to 79% of people with chronic heartburn report nighttime symptoms, based on a Gallup survey run for the American Gastroenterological Association. Three in four said it cost them sleep. Roughly 40% said it hurt the next day.
No dosing charts, no meal plans. Just what actually changes nighttime symptoms, and when the problem turns structural.

Why Does Acid Reflux Get Worse at Night?
Fewer episodes, longer damage. Nighttime reflux happens less often than daytime reflux, but each episode lasts far longer because nothing clears the acid.
Gravity Clocks Out
Upright, stomach contents stay put. Flat on your back, acid moves sideways as easily as down, and you lose the one force doing the work for free.
Your Stomach Empties Slower
Digestion slows during sleep. Food that clears in two hours sitting up lingers much longer when you’re flat. For some people that slowdown isn’t normal, and delayed stomach emptying is the real diagnosis.
Late Dinners Do the Most Damage
Eat at 9pm, lie down at 10pm, and your stomach is still full and still making acid. The American College of Gastroenterology suggests leaving two to three hours between dinner and bed, while grading that advice conditional with low-quality evidence.
The Valve Opens on Its Own
The lower esophageal sphincter should stay shut between swallows. It opens on its own several times an hour whether you’ve eaten or not. Those relaxations drive most nighttime reflux, which is why skipping dinner alone doesn’t fix it.
What Does Acid Reflux at Night Feel Like?
Often it doesn’t feel like heartburn at all. That’s the part people miss.
The classic version burns behind the breastbone, with a sour taste that wakes you. The missed version shows up as morning hoarseness, a cough that only happens after dark, or constant throat clearing. Those point toward silent reflux, where acid reaches the voice box without ever burning.
Patients come to us convinced they have allergies. They’ve already seen an ENT. Sometimes two.

How Do You Stop Acid Reflux at Night?
Position and timing beat diet changes for nighttime symptoms, and it isn’t close. Start there.
Put Three Hours Between Your Last Bite and Your Pillow
Best return on effort here. An empty stomach has less to push upward and sits under less pressure. Three hours is the target. Two beats one. Late-shift workers should move the big meal to midday.
Raise the Bed, Not Your Head
Stacking pillows bends your neck and compresses your belly, which makes things worse. Put six to eight inches of lift under the legs at the head end.
Actually, that framing isn’t quite right. Plenty of patients do fine with a wedge and never touch the frame. The real rule is that the incline starts at your hips.
Should You Sleep on Your Left Side?
Yes, and almost nobody does. Lying right-side down drops the stomach and esophagus junction below the pool of stomach contents. The left side flips that geometry and shortens acid contact time.
The Trigger Food List Everyone Repeats and Nobody Checks
Now the part other articles won’t say.
Chocolate, caffeine, spicy food, tomatoes, citrus, fried food. The same six everywhere. The ACG does suggest avoiding trigger foods, but grades that recommendation conditional with a low level of evidence. Lab studies on spicy food found little effect on sphincter pressure.
Food still matters, just not the way those lists claim. High-fat meals lower sphincter pressure and slow emptying at once. A JAMA study found 7 of 17 healthy volunteers had prolonged reflux episodes lying down after moderate alcohol, and none on the control night. Drop one food at a time and keep a log.
Belly Pressure Beats Body Weight
Abdominal weight pushes stomach contents upward, worse lying down. Waist measurement tracks reflux risk better than the scale. Tight waistbands do the same on a smaller scale.
Take the Pill Before Dinner, Not at Bedtime
Acid blockers work on pumps that meals activate. The ACG recommends a proton pump inhibitor 30 to 60 minutes before a meal, not at bedtime. Most people whose medication “stopped working” were taking it at the wrong hour.

When Does Acid Reflux at Night Become GERD?
Two or more episodes a week, or any symptom that wakes you, moves you out of occasional heartburn and into GERD.
Nighttime-dominant reflux tracks with more severe disease, not less. Longer acid contact raises the risk of esophageal inflammation and Barrett’s esophagus. Claims data from NIDDK put GERD diagnoses at 11.1% among privately insured Americans and 23.9% among Medicare enrollees.
Get evaluated for trouble swallowing, unexplained weight loss, or symptoms that haven’t moved after eight weeks on medication.
What Happens When Lifestyle Changes Aren’t Enough?
The default plan is a daily acid blocker refilled forever, with no objective testing. That’s where the system fails people.
If medication isn’t controlling nighttime symptoms, the problem usually isn’t acid production. It’s a barrier that failed mechanically, often from a hiatal hernia pulling the valve out of position.
At Tampa Bay Reflux Institute, Dr. Gopal Grandhige is a board-certified surgeon who starts with testing, not another prescription. pH monitoring, manometry, and endoscopy confirm whether symptoms are acid-driven before anyone discusses a procedure. SAGES surgical guidelines back repair for reflux medication can’t control.
Options run from rebuilding the valve with the top of the stomach, to a magnetic ring around the sphincter, to an incisionless repair through the mouth. Long-term data on the magnetic option shows 90.4% off daily acid blockers past five years. A 2025 multicenter study of incisionless repair reported 94% clinical success in year one.
One question almost no patient asks: how many of this exact procedure does the surgeon do each year? Ask it.
The One Change to Make Tonight
If you fix nothing else about acid reflux at night, move dinner earlier. Not by fifteen minutes. By two hours.
Timing costs nothing and works on the mechanism behind most nighttime episodes. Give it two weeks. Still waking at 3am? Then it isn’t your schedule. It’s your anatomy, and anatomy needs a different repair.
FAQs
Why is my acid reflux at night worse even though I take medication?
Timing is usually the issue. Proton pump inhibitors work on acid pumps that meals activate, so the ACG recommends taking them 30 to 60 minutes before a meal rather than at bedtime. If the timing is already right and symptoms persist, the problem may be mechanical rather than acid-related, which calls for pH testing and manometry.
How high should I raise the head of my bed for acid reflux at night?
Six to eight inches of lift at the head end is the range most often cited by Cleveland Clinic and Mayo Clinic, and the ACG supports raising the bed for nighttime GERD as a conditional recommendation with low-quality evidence. A full-torso wedge works as well as frame risers. Stacked pillows don’t, because they bend the neck and compress the abdomen.
Does sleeping on the left side really reduce reflux?
Yes. The ACG guideline notes that lying right-side down increases nighttime and post-meal reflux compared with lying left-side down, because the right side puts the stomach and esophagus junction below the pool of stomach contents. The left side also shortens how long acid stays in contact with the esophagus.
Do I have to give up coffee and chocolate?
Not automatically. Both relax the lower esophageal sphincter, but the ACG grades trigger-food avoidance as a conditional recommendation with a low level of evidence, and research on whether cutting them out improves symptoms stays mixed. Test one food at a time for two weeks and let your own log decide.
How long before bed should I stop eating?
Two to three hours is the standard recommendation from the ACG. High-fat meals are the worst offenders because they lower sphincter pressure and slow stomach emptying at the same time. Alcohol belongs in the same category: a JAMA study found 7 of 17 healthy volunteers had prolonged reflux episodes lying down after moderate alcohol, with none on the control night.
When should I see a reflux specialist instead of managing this at home?
See someone if symptoms happen twice a week or more, wake you from sleep, or haven’t improved after eight weeks on medication. Trouble swallowing, unexplained weight loss, or vomiting blood all mean go now. Nighttime-dominant reflux is linked to more severe disease, so waiting carries real cost.
Can a procedure fix nighttime reflux for good?
For the right candidate, yes. Long-term data on magnetic sphincter devices show 90.4% of patients free of daily acid blockers past five years, and a 2025 multicenter study of incisionless repair reported 94% clinical success within the first year. Candidacy depends on hiatal hernia size, esophageal motility, and whether testing confirms the symptoms are acid-driven.
An endoscopy cannot tell you if you have reflux. It can only tell you if you have complications of GERD.
