The signs you have acid reflux usually start with heartburn, a burning behind your breastbone after a big meal or when you lie flat. Then sour liquid in your mouth, trouble swallowing, a lump in the throat, a cough that won’t quit, a voice gone hoarse by afternoon. One rough night after a heavy dinner is normal. Twice a week or more isn’t.
Acid reflux happens when stomach contents flow backward into the esophagus, the tube running from your throat to your stomach. A ring of muscle at the bottom, the lower esophageal sphincter, should stay shut. When it weakens or opens at the wrong moment, acid gets through. Reflux that keeps happening and damages tissue is called GERD.
I’m Dr. Gopal Grandhige, a board-certified surgeon in Tampa. Most people who reach my office have self-treated for years. By then the question isn’t whether they have reflux. It’s how much damage they allowed.
Signs You Have Acid Reflux That Can’t Wait
Some reflux symptoms mean call your doctor this week. A few mean go now. Chest pain with shortness of breath, pain into your jaw or left arm, vomiting blood or coffee-ground material, black stools, food sticking when you swallow, or weight falling off without trying.
Any of those, go get seen.
Heartburn and a heart attack can feel identical, and neither of us can tell them apart at your kitchen table.
The quiet list matters just as much. NIDDK states plainly that not every adult with GERD gets heartburn. Some get only a hoarse voice, a raw throat each morning, or worn tooth enamel their dentist spots first.

What Triggers Acid Reflux?
Large meals, late eating, lying down after food, belly weight, smoking, and alcohol are the triggers with real evidence behind them. The famous food list (spicy, greasy, citrus, chocolate, mint, coffee, soda) is softer than anyone admits.
My contrarian take. The trigger-food list on your refrigerator is the most oversold advice in reflux care. The American College of Gastroenterology does suggest avoiding trigger foods, but its own guideline grades that conditional, on low-quality evidence. Weight loss earns a strong recommendation on moderate evidence. Patients never hear that gap.
A 2025 PMC meta-analysis found coffee drinkers had a small uptick in GERD rates and no real difference in reflux symptoms versus non-drinkers. Spicy food barely moves sphincter pressure in lab testing. Alcohol holds up. A review of 29 studies tied it to risk that climbs with intake.
Correction. That framing isn’t quite right. Food doesn’t cause reflux. Food exposes reflux you already have.
That difference decides your outcome. Cutting coffee helps mild, meal-driven symptoms. For a patient with a 4 cm hiatal hernia it changes nothing, because the problem is a gap in the diaphragm, not a cup of espresso.
What pays off, in order:
- Lose weight carried around the middle.
- Stop eating 2 to 3 hours before bed.
- Raise the head of the bed, not your pillows.
- Cut alcohol and tobacco.
- Keep a two-week food diary. Cut only what reliably burns you.

When Should You See a Doctor About Reflux?
Make the appointment if symptoms hit twice a week or more, if drugstore medicine stops working, if swallowing hurts, or if you’ve taken a daily acid blocker past eight weeks. Mayo Clinic draws that same twice-a-week line between reflux and GERD.
Eight weeks is the number to remember. ACG recommends an eight-week trial of a once-daily acid blocker taken before a meal. Still burning after that? You don’t need a stronger pill. You need testing.
Three patterns get mixed up constantly:

Silent reflux is the pattern I watch get mishandled most. Those patients bounce between ENT and pulmonology for a year before anyone looks at the esophagus.
Acid Reflux Treatments That Fix the Cause, Not the Burn
Treatment climbs a ladder. Lifestyle changes, then acid-blocking medicine, then testing, then a repair when medicine stops holding. Most people stall on step two for a decade.
Acid blockers work. They also leave a weak valve or a hernia exactly as they found it, which is why symptoms return the week you stop. The FDA cleared a new acid-blocker class in July 2024, and trial patients averaged 45% heartburn-free days against 28% on placebo. Better chemistry. Same mechanical problem.
Repairs have gotten quieter and more durable. Five-year FDA post-approval data in Annals of Surgery put roughly 90% of patients off daily acid medication. An incisionless repair through the mouth hit 94% clinical success at one year in a Gastrointestinal Endoscopy cohort. Wrapping the stomach stays most durable for large hernias.
Anyone recommending a procedure before pressure and pH testing is guessing.
Does Acid Reflux Go Away on Its Own?
Occasional acid reflux usually clears within a day or two. Reflux several times a week rarely does. A Global Burden of Disease analysis published in 2025 counted roughly 826 million people living with GERD in 2021, up from about 451 million in 1990. That curve doesn’t bend because people wait it out.

Why Reflux Gets Worse at Night
Reflux worsens at night because lying flat removes gravity and saliva production drops, so acid sits against the esophagus longer. Night symptoms reach up to 80% of GERD patients, by DelveInsight’s 2025 estimates.
Stacking pillows bends your neck and does nothing. Raise the whole head of the bed six inches on blocks. Finish dinner three hours before lying down.

Does drinking water help acid reflux?
Water by itself has not been shown to reduce reflux symptoms. In the Nurses’ Health Study II, swapping two daily servings of coffee, tea, or soda for two servings of water was tied to a modest drop in reflux symptom risk, while water intake on its own showed no association. The benefit comes from what the water replaces.
Is coffee bad for acid reflux?
For most people, no. A 2025 systematic review and meta-analysis found coffee drinkers had a small, statistically significant increase in GERD rates but no significant difference in actual reflux symptoms compared with non-drinkers. If coffee reliably burns you, cut it. If it doesn’t, the evidence doesn’t support quitting.
How long does acid reflux last?
A single episode of acid reflux usually lasts a few minutes to a couple of hours and settles once the stomach empties. Reflux that returns several times a week tends to persist until something changes, whether that’s weight, meal timing, medication, or a repair of the valve. Untreated chronic reflux can lead to esophagitis, strictures, and Barrett’s esophagus over time.
An endoscopy cannot tell you if you have reflux. It can only tell you if you have complications of GERD.
