The 10 worst foods that trigger acid reflux are fried food, chili peppers, citrus, tomato products, chocolate, coffee, alcohol, soda, mint, and raw onions. Every article on page one says some version of that.
Now the part nobody tells you. Cutting all ten won’t fix most people’s reflux. The American College of Gastroenterology rates trigger food avoidance as a conditional suggestion built on low-quality evidence. Weight loss gets a strong recommendation. Those ratings aren’t close.
I’ve sat across from patients who cut every item here and still woke up choking at 2 a.m.
What Are the 10 Worst Foods That Trigger Acid Reflux?
These ten loosen the valve at the base of your esophagus, slow your stomach, or irritate raw tissue. Most people react to two or three.
1. Fried and Fatty Foods
Start here. Fat slows stomach emptying and lowers pressure in the lower esophageal sphincter (LES), the ring holding acid down.
2. Chili Peppers and Hot Sauce
Capsaicin triggered heartburn in 28 of 31 people who already had GERD, but only 6 of 17 healthy controls. Spice amplifies reflux, rarely starts it.
3. Oranges, Lemons, and Grapefruit Juice
Citrus stings an inflamed esophagus on contact. Mornings are worst.
4. Tomato Sauce, Salsa, and Ketchup
Same problem, different fruit. Pasta night is a classic 10 p.m. offender.
5. Does Chocolate Really Cause Heartburn?
For some people, yes. Cocoa brings fat, caffeine, and methylxanthines, all nudging the valve open. But a square after lunch is nothing like half a bar at 11 p.m.
6. Coffee and Strong Tea
Decaf usually doesn’t help, which surprised me. New England Journal of Medicine research clocked acid output at 16.5 mEq/hour on decaf against 20.9 on regular coffee. The trouble is in the bean, not the caffeine.
7. Beer, Wine, and Liquor
Cleanest data here. A meta-analysis of 29 studies put drinkers at 1.48 times the odds of GERD, and 2.12 past three to five drinking days a week.
8. Do Soda and Sparkling Water Belong on This List?
Here I break with the list. A systematic review found no direct evidence that carbonated drinks worsen GERD. A later meta-analysis found a link at heavy intake. Soda earns its spot on sugar. Seltzer doesn’t.
9. Peppermint and Spearmint
Mint relaxes smooth muscle, exactly wrong for your LES. Daily peppermint tea drinkers showed double the odds of reflux disease.
10. Raw Onions and Garlic
A study fed heartburn sufferers a plain burger, then the same one with raw onion. Onion raised every reflux measure in that group. In people without reflux, nothing.

Why Do Certain Foods Set Off Reflux?
Foods set off acid reflux three ways: they relax the LES, slow digestion, or irritate tissue. But a weak valve is the root problem. Food exposes it.
Look at that onion study again. Same burger, two groups. One refluxed, one didn’t.
Belly weight presses on that valve. A hiatal hernia pulls the junction into the chest where it can’t seal. And when acid reaches your throat, it becomes silent reflux, which the food list barely touches.
How Do You Find Your Real Triggers in 2026?
Stop eliminating everything at once. Test one food at a time. In a study of 100 people with typical reflux, 85% pinned down at least one trigger, and symptoms improved two weeks later.
- Log for one week. Note the meal time and the burn time. The gap matters more than the food.
- Pull one suspect for two weeks. Start with fried food.
- Add it back and watch. Nothing happens? It was never your problem.
- Move to the next suspect. Repeat until the list is short.
- Fix timing and position. Stop eating three hours before bed, and raise the head of the bed six to eight inches. A wedge trial cut esophageal acid time from 21% to 15%.
Most people finish with three or four real triggers, not twenty.

What Should You Eat Instead?
Build meals around fiber, lean protein, and high-water produce. Fiber has the strongest evidence of anything on your plate.
In the Nurses’ Health Study II, researchers tracked 48,868 women and logged 10,674 new cases of acid reflux symptoms. The highest fiber group carried a relative risk of 0.75.
Now a correction. I used to tell patients that bananas and melon neutralize stomach acid. They don’t. Your stomach runs at a pH between 1.5 and 3.5, and no banana moves that. They just add no acid.
One more wrinkle. Fruit and vegetable fiber showed the effect. Cereal fiber didn’t. So the oatmeal advice everyone repeats has thinner support than the produce advice nobody does.

What I Tell Patients When the List Stops Working
Roughly one in three adults deals with reflux. Other estimates land closer to 18% to 28%. Either way, millions have already run this list into the ground.
You’ve cut your triggers. Lost weight. Raised the bed. Stopped eating after 7 p.m. And you’re still reaching for antacids at midnight.
The food was never your problem. You have a mechanical failure at the valve, and diet doesn’t repair mechanics. A surgical repair does. So does proper testing with pH monitoring and manometry, the step I’d fight for hardest. Skipping it is how people get the wrong operation.
