Signs it’s time to see a foregut specialist
You don’t need to want surgery to warrant an evaluation. You need one when reflux is persistent, escalating, or unexplained. Consider a specialist visit if any of these fit:
- You still get heartburn or regurgitation while taking a daily acid-blocking medication.
- You need more than one dose a day to feel normal.
- Your endoscopy came back “normal,” but the symptoms never stopped.
- You have a hiatal hernia larger than about 3 centimeters, or one that nobody has explained.
- You wake at night with acid in your throat or chest.
- You have a chronic cough, hoarseness, or throat-clearing that allergy and asthma treatment hasn’t fixed.
- You’ve had severe esophagitis, a stricture, or Barrett’s esophagus.
- You’ve been on reflux medication for years with no retesting.
- You want off long-term medication and want to know whether that’s realistic.
If several of these sound familiar, testing can tell you what’s actually driving them. Some of these symptoms turn out not to be reflux at all, which is exactly what the workup is built to catch.
What to expect, from your first call to recovery
Most patients are seen within two weeks and almost always within four. From the first visit to surgery, when surgery is the answer, usually runs four to eight weeks, depending on whether your testing is already done.
If you already have endoscopy, pH testing, manometry, and imaging, one visit is often enough to confirm the diagnosis and plan. If you don’t, the first visit covers review and test planning, and a second visit about four weeks later covers the results and the decision. Either way, nothing is rushed and nothing is decided before the data is in.
Bring your records. Email prior endoscopy and pathology reports, pH and manometry results, imaging, your medication history, and notes from your other doctors to info@tampareflux.com before your visit. In the current system, patients are the most reliable carriers of their own history, and having the records up front turns the consult into a real decision-making visit instead of a data hunt.
For Clearwater and Pinellas patients, the team consolidates studies into as few trips across the bay as possible and, when it can, arranges testing closer to home. Insurance authorization is handled by the office before anything is scheduled, so coverage is worked out ahead of time rather than after.
Fundoplication, LINX, and TIF are usually same-day-home procedures. Diet advances over a structured few weeks, and closer to two or three months to fully expand after a wrap. Silent-reflux symptoms are the slow ones: throat and voice complaints often take four to six months to settle, while classic heartburn can improve almost right away.
Meet Dr. Gopal Grandhige

Dr. Gopal Grandhige is a board-certified general surgeon who has focused on foregut surgery, reflux, hiatal hernias, achalasia, and gastroparesis since 2009. He founded Tampa Bay Reflux Center in 2009 and Tampa Bay Reflux Institute in 2022.
He earned a biology degree from Johns Hopkins University, attended medical school at the University of Michigan, and completed a general surgery residency plus fellowships in foregut and minimally invasive surgery, and in burn and critical care, at Yale-New Haven Hospital. He is a Fellow of the American College of Surgeons (FACS), a member of the Society of American Gastrointestinal and Endoscopic Surgeons (SAGES), and a founding member of the American Foregut Society. He performs all surgeries at HCA South Tampa Hospital, alongside a dedicated physician assistant who assists on every case and stays reachable after the operation.

Reflux and hiatal hernia are mechanical problems, and they respond to a mechanical fix, but only when the diagnosis is right.
