Why your reflux keeps coming back
Most reflux that persists on daily medication is a mechanical problem, not an acid problem. The barrier that normally keeps stomach contents down has three parts: the lower esophageal sphincter, the diaphragm around it, and the angle where the esophagus meets the stomach. When that barrier weakens, often from a hiatal hernia, stomach contents move up into the esophagus, throat, or airway. The National Institute of Diabetes and Digestive and Kidney Diseases notes that GERD develops when that sphincter becomes weak or relaxes when it should not.
Acid-suppressing medication lowers the acidity of what refluxes. It does not strengthen the sphincter, repair a hernia, or restore the diaphragm. So the reflux keeps happening, it is simply less acidic, and it still carries bile, pepsin, and other stomach contents that can irritate tissue. This is why so many people feel better during the day but still wake at night with a cough, a sour taste, or a burning throat, and why symptoms return the moment a dose is missed. Medication turns down the burn. It does not stop the leak.

The four anti-reflux procedures, and how the right one gets chosen
There is no single best reflux operation. The right one depends on your anatomy, the strength of your esophagus, the pattern of your symptoms, and your own priorities, and in many cases the right answer is no surgery at all. Dr. Grandhige is the only board-certified surgeon in the Tampa Bay area who performs fundoplication, LINX, and TIF regularly, with more than 600 fundoplications, more than 600 LINX procedures, and more than 200 TIF procedures behind him. He is adding RefluxStop in 2026, which will make him the only surgeon in the area offering all four.
A fundoplication wraps the upper stomach around the lower esophagus to rebuild the barrier. It is the most durable option, it repairs the hernia at the same time, and it has decades of outcome data, though depending on the wrap it can limit belching and vomiting.
LINX is a ring of magnetic beads placed around the sphincter. It preserves normal anatomy, usually keeps the ability to burp and vomit, and is removable, but it needs a reasonably strong esophagus and carries a higher chance of early swallowing difficulty.
TIF is done through the mouth with no incisions and no hernia repair, which makes it a fit for mild to moderate reflux with little or no hernia, and a poor fit for large hernias, reflux complications, or patients who are obese. It is the least durable of the options, with a failure rate Dr. Grandhige quotes at about 2% per year, and he describes it as the best endoscopic procedure available rather than a permanent fix.
RefluxStop reinforces the reflux barrier without wrapping the esophagus, and is the newest of the four.
The choice is never made because a procedure exists. Dr. Grandhige confirms reflux is real, maps the anatomy, tests esophageal strength, matches which symptoms should improve to what the testing shows, weighs your goals such as durability or keeping the ability to burp, and only then selects a procedure, or recommends none. Because he performs all of them, that decision is driven by what fits you, not by the one operation a surgeon happens to do.
Honest results, and when surgery is not the answer
In appropriately selected patients with typical reflux and good esophageal function, more than 95% get strong symptom relief and stop their daily reflux medication. That number is not an accident. It comes from testing first, selecting carefully, and matching the procedure to the anatomy. Throat-symptom patients selected with dual-channel testing improve at roughly 80%. The results are honest about limits, too. Anti-reflux procedures are functional repairs. Like a joint replacement, they are subject to wear and can occasionally need revision, and TIF in particular trades durability for being incisionless.
Just as important is when not to operate. Symptoms that look like reflux can come from esophageal hypersensitivity, motility disorders, functional chest pain, or ENT and lung conditions, none of which surgery fixes. Dr. Grandhige turns these patients away from the operating room, and says plainly that they are often the most frustrated people he sees, but that they would be unhappier after a surgery that did not help. Many of his most satisfied patients are the ones he talked out of surgery. Referring gastroenterologists, ENT physicians, and pulmonologists across Tampa Bay, including the Pinellas side, send patients here specifically for that kind of evaluation.

Why Clearwater patients make the drive
Clearwater patients travel to the South Tampa office, about 30 to 45 minutes across the Courtney Campbell Causeway, because foregut outcomes track specialization and consistency more than proximity. Dr. Grandhige works only on the esophagus, diaphragm, and stomach, and performs every operation at one hospital, HCA South Tampa, with the same operating room team and his own dedicated physician assistant in every case and in follow-up. That continuity is hard to match for a surgeon who operates occasionally across several hospitals with rotating teams. As a founding member of the American Foregut Society, he practices in the subspecialty where surgeons and gastroenterologists collaborate specifically on foregut disease.
