New Gastroparesis Treatments In St. Petersburg, FL

Delayed stomach emptying that hasn’t responded to diet changes and medication can often be treated with an incisionless procedure called G-POEM. Dr. Gopal Grandhige, a fellowship-trained foregut surgeon, performs it in South Tampa, about a 30-minute drive from St. Petersburg, and is one of only a handful of surgeons in the Tampa Bay area who does. Testing comes first, and he recommends the procedure only when it is likely to help.

What “new” gastroparesis treatment means for St. Petersburg patients

The biggest change in gastroparesis care is the move away from open stomach surgery toward an incisionless procedure done through the mouth, called endoscopic pyloromyotomy, or G-POEM. For many St. Petersburg patients who have cycled through diet changes and promotility medication without relief, it is the first option that treats the mechanical problem instead of masking symptoms.

Here is the honest framing up front: these procedures reduce symptoms, they do not cure gastroparesis. Gastroparesis, also called delayed gastric emptying, slows or stops food moving from the stomach into the small intestine even though nothing is physically blocking it, according to the National Institute of Diabetes and Digestive and Kidney Diseases. It is uncommon, roughly 10 men and 40 women per 100,000 people, and diabetes is its most common known cause. Endoscopic pyloromyotomy (G-POEM), explained

G-POEM opens the pylorus, the muscular valve between the stomach and the small intestine, so the stomach can empty. Using a flexible endoscope passed through the mouth, Dr. Grandhige creates a small tunnel under the pyloric muscle and divides that muscle with an endoscopic knife. There are no incisions on the abdomen. Most patients go home the same day or the next.

The evidence behind it is specific. In a randomized, sham-controlled trial, 71 percent of patients treated with G-POEM had at least a 50 percent drop in symptom scores at six months, compared with 22 percent who received a sham procedure. Among patients with diabetic gastroparesis, the success rate was 89 percent. Set against the older device-based option, a gastric electrical stimulator, a 2025 multicenter analysis reported by the American College of Surgeons found G-POEM had far shorter operating times, less blood loss, and a much shorter hospital stay, about half a day versus more than two.

Because it is endoscopic, recovery is quicker than with open surgery, and patients follow a staged diet as the stomach adjusts. G-POEM is increasingly used as an early surgical option rather than a last resort.

The other gastroparesis treatments Dr. Grandhige offers

G-POEM is not the only option, and choosing the right treatment for the patient matters more than defaulting to one procedure. When someone is not a good endoscopic candidate, Dr. Grandhige performs pyloroplasty, a robotic or laparoscopic version of the same pyloric opening. For severe cases with weight loss or malnutrition, a feeding tube can restore nutrition before any definitive procedure is considered. In rare cases that have failed every other therapy, subtotal gastrectomy is a last option. Diet changes and promotility or anti-nausea medication stay part of the plan for many patients, often alongside a procedure rather than in place of one.

Because gastroparesis can drive severe acid reflux (GERD) and overlaps with other foregut problems, being seen by a surgeon who treats the whole area helps. Dr. Grandhige also treats achalasia, a swallowing disorder in which the valve at the bottom of the esophagus fails to relax, using a related endoscopic myotomy.

Who is a candidate for these procedures

Not everyone with gastroparesis benefits from a procedure, and candidacy is decided by objective testing, not symptoms alone. The main test is a four-hour gastric emptying study: you eat a light meal containing a trace of radioactive material, and a scanner measures how fast your stomach empties. An upper endoscopy is often done as well, partly to rule out a blockage and partly because undigested food after an overnight fast points toward the diagnosis.

Dr. Grandhige’s approach is deliberate: confirm delayed emptying with testing, understand the cause, then recommend a procedure only when it is likely to help. That restraint is the point. Patients who are unlikely to improve are told so directly, which spares them a procedure that would not have worked. The honest version of the outcome is this: G-POEM and its alternatives can reduce symptoms substantially in the right patient, but no procedure resolves every symptom, and durability varies from person to person.
